摘 要:张机(字仲景)所著的《伤寒论》是中医学的一颗璀璨的明珠,小建中汤作为《伤寒论》的经典方剂之一,其君药饴糖性味甘、温,归脾、胃、肺经,在经方中起到温中补虚、和里缓急的功效。但是,近些年,由于药房普遍不能供应,有报道发现在临床使用小建中汤时常存在去掉饴糖或者采用其他替代品的使用方法。为了更全面地了解小建中汤中的饴糖应用现状及不同替代方法对小建中汤疗效的影响,笔者特对涉及小建中汤的理论及临床应用研究的学术期刊、学位论文、会议论文等进行了检索,整合筛选出了最新的以及高参考价值的一些参考文献,排除年代久远以及参考价值较小的参考文献后,发现大量学者均认为饴糖在小建中汤中具有重要作用,不建议在小建中汤加减的临床应用中去掉饴糖。针对目前很多药房没有饴糖的客观现状,有研究认为,山药与麦芽一起糖化发酵为麦芽糖后可替代饴糖发挥其在小建中汤中温补中脏的作用,但仍需进一步研究探寻得糖率高且操作简单、切实可行的煎煮方法;也有研究认为,葡萄糖与饴糖在小建中汤方中作用相近,确无饴糖时可以替代使用。笔者现对近年小建中汤中的饴糖应用研究现状和进展进行阐述,以期为临床实践及进一步研究提供参考。Shanghan Lun(《伤寒论》)written by ZHANG Ji is a shining pearl of traditional Chinese medicine.Xiaojianzhong decoction(小建中汤)is one of the classic prescriptions in Shanghan Lun,Yitang(Cerealose)is the monarch drug in the prescription;it is sweet and warm in property and returns to the spleen,stomach,and lungs meridians,and has the effect of warming the middle-jiao,tonifying,and harmonizing the body.However,due to its widespread unavailability in pharmacies,it has been reported that in recent years of clinical use of Xiaojianzhong decoction often involves the use of Yitang or other substitutes.In order to gain a more comprehensive understanding of the current status of the application of Yitang in Xiaojianzhong decoction,and the effects of different alternative methods on its efficacy,the author conducts a search of academic journals,academic dissertations,and conference papers on the theoretical and clinical application of Xiaojianzhong decoction,integrates and screens out some of the latest references with high reference value,excluding those which are old and of the lesser reference value,and it is found that a large number of scholars consider that Yitang plays an important role in Xiaojianzhong decoction and it is not recommended to remove Yitang in the clinical application of Xiaojianzhong decoction.In view of the objective situation that Yitang is not available in many pharmacies at present,some studies believe that after saccharification and fermentation of Shanyao(Yam)with Maiya(Malt)into maltose,maltose can replace Yitang and play its role in warming the middle-jiao.However,further research is needed to explore the decocting method with high sugar yield,simple and feasible operation.There are also studies which have proved that glucose and Yitang have similar effects in Xiaojianzhong decoction,and glucose can be used when there is no Yitang.In this paper,the author reviews the research status and progress of the application of Yitang in Xiaojianzhong decoction in recent years,in order to prov
