摘  要:目的观察夹脊穴埋线通过调控脑源性神经营养因子(BDNF)/原肌球蛋白受体激酶B(TrkB)信号通路对大鼠脊髓损伤后自噬的影响。方法将60只雄性Wistar大鼠随机分为假手术组、模型组和穴位埋线组,每组20只。模型组和穴位埋线组大鼠采用重物坠落法建立脊髓损伤模型,穴位埋线组于造模成功后取夹脊穴给予简易埋线,每7 d埋线1次,模型组不予干预。分别于实验第7天、第28天,应用Basso-Beattie-Bresnahan(BBB)评分和斜坡实验评估大鼠的运动功能和平衡功能,ELISA法检测血清中炎症细胞因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)]水平,免疫组织化学染色观察脊髓组织中BDNF的阳性表达情况,Western blot法检测脊髓组织中自噬标记蛋白(Beclin-1、LC3-B)和BDNF/TrkB通路蛋白(BDNF、TrkB、p-TrkB)的表达情况,RT-PCR法检测脊髓组织中BDNF、p-TrkB mRNA表达情况。结果实验第7天、第28天,与假手术组比较,模型组大鼠BBB评分及斜坡实验角度均明显降低(P均<0.05),血清TNF-α、IL-1β、IL-6水平均明显升高(P均<0.05),脊髓组织中BDNF阳性细胞数和Beclin-1、LC3-B、BDNF、p-TrkB蛋白相对表达量及BDNF、p-TrkB mRNA相对表达量均明显增加(P均<0.05);与模型组比较,穴位埋线组大鼠BBB评分及斜坡实验角度均明显升高(P均<0.05),血清TNF-α、IL-1β、IL-6水平均明显降低(P均<0.05),脊髓组织中BDNF阳性细胞数和Beclin-1、LC3-B、BDNF、p-TrkB蛋白相对表达量及BDNF、p-TrkB mRNA相对表达量均明显增加(P均<0.05)。结论夹脊穴埋线可激活大鼠脊髓损伤后自噬活动,促进运动和神经功能恢复,其机制与激活BDNF/TrkB通路,提高BDNF活性和TrkB磷酸化水平相关。Objective It is to observe the effect of catgut embedding at“Jiaji acupoint”on autophagy after spinal cord injury(SCI)in rats via regulating brain derived neurotrophic factor(BDNF)/tropomyosin receptor kinase B(TrkB)signaling pathway.Methods Sixty male Wister rats were randomly divided into sham operation group,model group,and acupoint catgut embedding group,with 20 rats in each group.The rats in the model group and acupoint thread embedding group injuried by heavy object falling method to establish spinal cord injury models.After successful modeling,the acupoint thread embedding group was treated with simple thread embedding at Jiaji acupoints,once every 7 days.The model group was not given the intervention.On the 7th and 28th day of the experiment,the motor and balance functions of the rats were respectively assessed by Basso-Beattie-Bresnahan(BBB)score and slope test,the serum levels of inflammatory cytokines[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-1β(IL-1β)]were detected by ELISA,and the positive expression of BDNF in spinal cord tissues was detected by immunohistochemical staining,the positive expressions of autophagy marker proteins(Beclin-1,LC3-B)and BDNF/TrkB pathway proteins(BDNF,TrkB,p-TrkB)were detected by Western blot,and the expressions of BDNF,p-TrkB mRNA in spinal cord tissues were detected by RT-PCR.Results On the 7th and 28th days of the experiment,compared with the sham operation group,the BBB score and slope experimental angle of the rats in the model group were significantly reduced(all P<0.05),the levels of TNF-α,IL-1βand IL-6 in serum were significantly increased(all P<0.05),the number of BDNF positive cells and the relative expressions of Beclin-1,LC3-B,BDNF,p-TrkB proteins,as well as the relative expressions of BDNF and p-TrkB mRNA in spinal cord tissue were significantly increased(all P<0.05).Compared with the model group,the BBB score and slope experimental angle of the rats in the acupoint catgut embedding group were significantly increased(all P<0.0
