针药结合法协同静脉溶栓治疗急性脑梗死的临床观察

摘　　要：目的探讨针药结合法协同静脉溶栓治疗急性脑梗死(ACI)的临床疗效。方法将2021年1月至2022年12月该院符合标准的224例ACI患者按就诊的先后顺序纳入结合组、中药组、针刺组和对照组,各56例。对照组给予静脉溶栓治疗,中药组在对照组治疗基础上给予解语丹加减方治疗,针刺组在对照组治疗基础上给予醒脑开窍针刺法治疗,结合组在对照组治疗基础上给予解语丹加减方与醒脑开窍针刺法治疗;4组治疗均5 d为1个疗程,共3个疗程。结果4组中医证候评分、炎性细胞因子与血流变学指标较治疗前均有不同程度的降低,差异均有统计学意义(P<0.05),且结合组与中药组的降低程度更为明显,差异均有统计学意义(P<0.05);在基本痊愈方面,结合组、中药组、针刺组均优于对照组,差异均有统计学意义(P<0.05)。结论采用针药结合法协同静脉溶栓治疗ACI的临床应用疗效显著。Objective To explore the clinical effect of acupuncture combined with medicine combined with intravenous thrombolysis in the treatment of acute cerebral infarction(ACI).Methods A total of 224 ACI patients who met the standards were included in the combination group,the traditional Chinese medicine(TCM)group,the acupuncture group,and the control group in the order of treatment,with 56 cases in each group.The control group received intravenous thrombolysis,while the TCM group received Jieyu Dan Jiajian Fang on the basis of the control group.The acupuncture group received Xingnao Kaiqiao Acupuncture on the basis of the control group.The combination group received Jieyu Dan Jiajian Fang and Xingnao Kaiqiao Acupuncture on the basis of the control group.All four groups were treated for five days as one course of treatment,with a total of three courses of treatment.Results In the comparison of TCM syndrome score,inflammatory factors and blood rheology indicators,the four groups had different degrees of reduction compared with those before treatment(P<0.05),and the reduction was more obvious in the combination group and the TCM group(P<0.05).In terms of basic recovery,the combination group,the TCM group,and the acupuncture group were all better than the control group(P<0.05).Conclusion The clinical application of acupuncture and medicine combined with intravenous thrombolysis in the treatment of ACI has significant curative effect.
