血液学指标联合血红蛋白电泳检测对地中海贫血和缺铁性贫血的诊断价值

摘　　要：目的:探讨血液学指标检测联合血红蛋白电泳检测对地中海贫血和缺铁性贫血的诊断价值。方法:选取2021年1月—2021年9月汕头市妇幼保健院检查的245例患者分为三组,其中地中海贫血组135例(α-地中海贫血组90例,β-地中海贫血组45例);缺铁性贫血组30例和正常组80例。每位患者均进行血常规和血红蛋白电泳检测。结果:地中海贫血组患者和缺铁性贫血组患者的平均红细胞体积(MCV)、平均红细胞血红蛋白量(MCH)均比正常组患者小,差异有统计学意义(P<0.01);缺铁性贫血组患者的红细胞分布宽度(RDW)比正常组患者小,差异有统计学意义(P<0.01);地中海贫血组患者的血红蛋白A_(2)(HbA_(2))比正常组患者小,差异有统计学意义(P<0.01);MCV、MCH、RDW对地中海贫血组患者和缺铁性贫血组患者诊断的灵敏度比较,差异有统计学意义(χ^(2)=19.215、6.261、32.154,P<0.01)。结论:MCV、MCH、RDW等血液学指标联合血红蛋白电泳检测对地中海贫血和缺铁性贫血诊断。Objective:To investigate the diagnostic value of hematological index test combined with hemoglobin electrophoresis test for thalassemia and iron deficiency anemia.Methods:A total of 245 patients examined in hospitals from January 2021 to September 2021 were selected and divided into 135 in the thalassemia group(90 in theα-thalassemia group,45 in theβ-thalassemia group),30 in the iron deficiency anemia group and 80 in the normal group.Routine blood and hemoglobin electrophoresis tests were performed in each patient.Results:MCV and MCH were smaller in thalassemia and iron deficiency anemia than in the normal group,and the difference was statistically significant(P<0.01).The RDW of the iron deficiency anemia group was smaller than that of the normal group,and the difference was statistically significant(P<0.01).HbA_(2) was smaller in the thalassemia group than in the normal group,with a statistically significant difference(P<0.01).The differences in MCV,MCH,and RDW sensitivity between the thalassemia group and the iron deficiency anemia group were statistically significant(χ^(2)=19.215,6.261,32.154,P<0.01).Conclusion:Hematological indices such as MCV,MCH,RDW combined with hemoglobin electrophoresis test for the diagnosis of thalassemia and iron deficiency anemia.
