探究滤泡辅助性T细胞在溃疡性结肠炎中的作用

摘　　要：目的:对滤泡辅助T细胞(Tfh)在溃疡性结肠炎(UC)临床患者中的表达水平及作用机制进行探索。方法:选取2018年1月—2019年12月广州医科大学附属第二医院25例UC患者临床外周血标本作为实验组,另选取25名健康成人外周血标本作为对照组,采用流式细胞术检测外周血中Tfh细胞的比例,进一步分析Tfh细胞上游关键转录因子Bcl6的表达,并检测血浆中重要细胞因子白介素21(IL-21)的含量。结果:相较于对照组,实验组患者外周血中Tfh细胞比例显著上升;实验组患者调控Tfh功能的关键转录因子Bcl6的表达增高,上游细胞因子IL-21的含量显著增加,且IL-21的含量与Tfh的比例有显著相关性,差异有统计学意义(R_(2)=0.5498,P<0.001)。结论:Tfh细胞在溃疡性结肠炎疾病的发生发展中可能发挥重要调控作用,上述研究结果为Tfh细胞在自身免疫性疾病中的治疗可提供潜在的药物靶点。Objective:To explore the expression level and mechanism of action of follicular helper T cells(Tfh)in clinical patients with ulcerative colitis(UC).Methods:25 clinical peripheral blood specimens of UC patients in the hospital from January 2018 to December 2019 were selected as the experimental group,and another 25 healthy adult peripheral blood were selected as the control group.The percentage of Tfh cells in peripheral blood was measured by flow cytometry,and the expression of Bcl6,a key transcription factor upstream of Tfh cells was further analyzed,and the levels of interleukin 21(IL-21),an important cytokine were measured in plasma.Results:Compared to healthy controls,the proportion of Tfh cells in the peripheral blood of patients with colitis showed a significant increase.The expression of Bcl6,a key transcription factor that regulates Tfh function,was upregulated in UC patients.Upstream cytokine IL-21 showed a statistically significant increase and the difference between the level of IL-21 and the proportion of Tfh was statistically significant(R_(2)=0.5498,P<0.001).Conclusion:Tfh cells may play an important regulatory role in the development of ulcerative colitis disease,and the above findings may provide potential drug targets for the treatment of Tfh cells in autoimmune diseases.
