原发性闭角型青光眼患者术后发生恶性青光眼的影响因素

摘　　要：目的:分析原发性闭角型青光眼(PACG)患者术后发生恶性青光眼的影响因素。方法:回顾性分析2020年9月至2023年2月该院收治的35例PACG术后1个月发生恶性青光眼患者的临床资料,设为发生组。回顾性分析同期该院收治的35例PACG术后1个月未发生恶性青光眼患者的临床资料,设为未发生组。比较两组临床资料,采用多因素Logistic回归分析PACG术后发生恶性青光眼的影响因素。结果:发生组年龄<50岁、合并糖尿病、术前持续高眼压、眼轴长度<22 mm占比均显著高于未发生组,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,年龄<50岁、合并糖尿病、术前持续高眼压、眼轴长度<22 mm均为PACG患者术后发生恶性青光眼的危险因素(OR>1,P<0.05)。结论:年龄<50岁、合并糖尿病、术前持续高眼压、眼轴长度<22 mm均为PACG患者术后发生恶性青光眼的危险因素。Objective:To analyze influencing factors of malignant glaucoma in postoperative patients with primary angle-closure glaucoma(PACG).Methods:The clinical data of 35 patients with malignant glaucoma 1 month after PACG in this hospital from September 2020 to February 2023 were retrospectively analyzed and they were set as the occurrence group.The clinical data of 35 patients with PACG who did not develop malignant glaucoma 1 month after surgery were retrospectively analyzed and they were set as the non-occurrence group.The clinical data of the two groups were compared.Multivariate Logistic regression analysis was used to analyze the influencing factors of malignant glaucoma after PACG.Results:The proportion of the patients in the occurrence group with age<50 years old,diabetes,persistent high intraocular pressure before surgery,and axial length<22 mm were significantly higher than those in the non-occurrence group,and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that age<50 years old,diabetes,preoperative persistent intraocular hypertension,and ocular axis length<22 mm were all risk factors for malignant glaucoma in the patients after PACG(OR>1,P<0.05).Conclusions:Age<50 years old,diabetes,persistent high intraocular pressure before surgery,and axial length<22 mm are the risk factors for malignant glaucoma after PACG.
