外周血MGP mRNA水平与溃疡性结肠炎并发CMV感染严重程度的相关性

摘　　要：目的探究外周血基质Gla蛋白(MGP)mRNA水平与溃疡性结肠炎(UC)患者巨细胞病毒(CMV)感染严重程度的相关性。方法选择2020年3月-2023年3月衢州市人民医院收治的83例UC患者,包括CMV感染29例(CMV组),无CMV感染54例(无CMV组),另选择50名健康体检人群为对照组,常规检测实验室指标,采用实时荧光定量逆转录-聚合酶链式反应(RT-PCR)方法检测外周血MGP mRNA表达量,分析各组指标差异。结果CMV组病程长于无CMV组,疾病严重程度重度比例、糖皮质激素和免疫抑制剂的使用率、糖皮质激素抵抗率高于无CMV组(P<0.05);CMV组白细胞(WBC)计数、红细胞沉降率(ESR)、高敏C-反应蛋白(hs-CRP)、MGP mRNA水平高于对照组、无CMV组(P<0.05),无CMV组WBC、ESR、hs-CRP、MGP mRNA水平高于对照组(P<0.05);CMV组重度患者WBC、ESR、hs-CRP、MGP mRNA水平均高于中度患者(P<0.05);UC合并CMV感染患者外周血MGP mRNA水平与WBC、ESR、hs-CRP、病情严重程度均呈正相关(P<0.05)。结论UC患者发生MCV感染过程中,外周血MGP mRNA表达水平增加,且与疾病活动度有关,可作为评估疾病严重程度的参考指标。OBJECTIVE To explore the correlation between mRNA level of matrix Gla protein(MGP)in peripheral blood and severity of cytomegalovirus(CMV)infection in patients with ulcerative colitis.METHODS Total of 83 patients with UC admitted to Quzhou People′s Hospital were between Mar 2020 and Mar 2023 were recruited,including 29 cases with CMV infection as the CMV group and 54 cases without CMV infection as the non-CMV group.Another 50 healthy volunteers under physical examination were enrolled as the control group.The laboratory indexes were tested routinely,and mRNA expression of MGP in peripheral blood was detected by real-time fluorescence quantitative reverse transcription-polymerase chain reaction(RT-PCR)assay.The differences in the indexes among groups were compared.RESULTS The course of disease in the CMV group was longer than that in the non-CMV group,and the proportion of severe disease,use rates of glucocorticoids and immunosuppressants and glucocorticoid resistance rate were higher than those in the non-CMV group(P<0.05).White blood cell count(WBC),erythrocyte sedimentation rate(ESR),and levels of high-sensitivity C-reactive protein(hs-CRP)and MGP mRNA in the CMV group were higher than those in the control group and non-CMV group(P<0.05),and the mRNA levels of WBC,ESR,hs-CRP and MGP in the non-CMV group were higher than those in the control group(P<0.05).The levels of WBC,ESR,hs-CRP and MGP mRNA in severe patients were higher than those in moderate patients of the CMV group(P<0.05).MGP mRNA level in peripheral blood of UC patients complicated with CMV infection was positively correlated with WBC,ESR,hs-CRP and severity of disease(P<0.05).CONCLUSION In the process of MCV infection in patients with UC,the expression of peripheral blood MGP mRNA increases,which is related to disease activity and can be used as a reference indicator for evaluating disease severity.
