阿加曲班联合抗血小板药物治疗后循环穿支动脉粥样硬化疾病型急性脑梗死患者的效果分析

摘　　要：目的:探讨阿加曲班联合抗血小板药物治疗后循环穿支动脉粥样硬化疾病(BAD)型急性脑梗死患者的效果。方法:选取2021年1月—2023年1月于上海市第七人民医院就诊的后循环BAD型急性脑梗死患者90例,经随机数字表法分为对照组和观察组,各45例。对照组给予常规抗血小板药物治疗,观察组于对照组治疗方案基础上给予阿加曲班治疗。观察两组神经损伤相关因子、脑血管储备功能(CVRC)、不良反应。结果:治疗后,观察组神经损伤相关因子水平均较对照组更低,差异均有统计学意义(P<0.05);治疗后,观察组收缩期峰值血流速度高于对照组,阻力指数较对照组更低,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:阿加曲班联合抗血小板药物应用于后循环BAD型急性脑梗死患者的治疗,可以有效减轻神经损伤,改善CVRC,且不会增加不良反应的发生。Objective:To evaluate the effect of Argatroban combined with antiplatelet drug in the treatment of patients with posterior circulation branch atherosclerosis disease(BAD)type acute cerebral infarction.Method:A total of 90 patients with posterior circulation BAD type acute cerebral infarction treated in Shanghai Seventh People's Hospital from January 2021 to January 2023 were selected and divided into control group and observation group according to random number table method,with 45 cases in each group.The control group was treated with conventional antiplatelet drug,and the observation group was treated with Argatroban based on the treatment regimen of the control group.Nerve injury related factors,cerebrovascular reserve capacity(CVRC)and adverse reactions were observed in the two groups.Result:After treatment,the levels of nerve injury related factors in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the peak systolic blood flow velocity in observation group was higher than that in control group,and the resistance index in observation group was lower than that in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Argatroban combined with antiplatelet drug in the treatment of patients with posterior circulation BAD type acute cerebral infarction can effectively reduce nerve injury and improve CVRC,and does not increase the occurrence of adverse reactions.
