重组人干扰素α-2b凝胶用于宫颈柱状上皮异位合并HPV感染治疗的效果及对HPV载量的影响

摘　　要：目的观察重组人干扰素α-2b凝胶用于宫颈柱状上皮异位合并人乳头瘤病毒(HPV)感染治疗的效果及对HPV载量的影响。方法选取2020年1月—2021年12月泉州市中医院收治的宫颈柱状上皮异位合并HPV感染患者120例,采用随机数字表法分为干扰素联合组和微波组,各60例。微波组接受微波治疗,干扰素联合组在微波组基础上加用重组人干扰素α-2b凝胶治疗,治疗时间为3个月。比较2组治疗效果、临床症状恢复状况(阴道流血时间、阴道排液时间、创面愈合时间),治疗前后炎性因子[肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-10(IL-10)]水平与HPV载量变化以及HPV转阴率。结果干扰素联合组患者治疗总有效率为98.33%,高于微波组的88.33%(χ^(2)=4.821,P=0.028);干扰素联合组阴道流血时间、阴道排液时间、创面愈合时间均短于微波组(P均<0.01);治疗3个月后,2组TNF-α、IL-6、IL-10水平与HPV载量均较治疗前下降,且干扰素联合组低于微波组(P均<0.01);干扰素联合组患者HPV转阴率为96.67%,高于微波组的76.67%(χ^(2)=10.385,P=0.001)。结论重组人干扰素α-2b凝胶用于宫颈柱状上皮异位合并HPV感染治疗的效果肯定,可有效改善临床症状,减轻炎性反应并降低HPV病毒载量。Objective To observe the effect of recombinant human interferonα2b gel in the treatment of columnar ectopy combined with HPV infection and its impact on HPV viral load.Methods A total of 120 patients with columnar ectopy complicated with HPV infection admitted to Quanzhou Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected.The patients were randomly divided into observation group and control group,with 60 cases in each group.The control group received microwave treatment,and the observation group was treated with recombinant human interferonα2b gel on the basis of the control group.The duration of medication was 3 months.The treatment effect,clinical symptoms recovery(vaginal bleeding time,vaginal drainage time,wound healing time),inflammatory factors(TNF-α,IL-6,IL-10)levels and HPV viral load changes before and after treatment,as well as HPV negative conversion rate were compared between the two groups.Results The total effective rate of the observation group was 98.33%,which was higher than 88.33%of the control group(χ^(2)=4.821,P=0.028).The vaginal bleeding time,vaginal drainage time and wound healing time of the observation group were shorter than those of the control group(all P<0.01).After treatment,the levels of TNF-α,IL-6,IL-10 and HPV viral load in the two groups were decreased compared with those before treatment,and all items in the observation group were lower than those in the control group(all P<0.01).The HPV negative conversion rate of the observation group was 96.67%,which was higher than 76.67%of the control group(χ^(2)=10.385,P=0.001).Conclusion The effectiveness of recombinant human interferonα2b gel in the treatment of columnar ectopy combined with HPV infection is certain,which can effectively improve the clinical symptoms of patients,reduce inflammatory reactions,and reduce the HPV viral load.
