摘 要:目的 探讨溃疡性结肠炎(UC)患者肠道菌群失调及血清Toll样受体4(TLR4)、髓过氧化物酶(MPO)的表达与疾病活动度的关系。方法 选择2020年11月至2023年1月中国人民武装警察部队河南省总队医院收治的240例UC患者为研究对象,根据疾病的严重程度分为临床缓解组、轻度组、中度组及重度组,每组60例,同时纳入60例健康体检者作为健康对照组。比较各组间肠道菌群,血清TLR4、MPO水平,并进行Pearson相关分析。结果 与健康对照组比较,重度组肠道双歧杆菌、真杆菌含量减少(P<0.05),但肠球菌属含量无明显变化。临床缓解组双歧杆菌、真杆菌含量高于重度组(P<0.05),但仍低于健康对照组(P<0.05),肠球菌属含量与健康对照组比较,差异未见统计学意义(P>0.05)。与健康对照组比较,轻度组、中度组以及重度组血清TLR4、MPO水平表达逐渐增加,其中重度组表达量明显升高(P<0.05)。与健康对照组比较,重度组免疫球蛋白A(IgA)、免疫球蛋白M(IgM)以及免疫球蛋白G(IgG)表达量减少(P<0.05),临床缓解组IgA、IgM以及IgG表达量高于重度组(P<0.05),但仍低于健康对照组(P<0.05)。Pearson相关性分析显示,健康对照组TLR4与MPO表达无相关性(P>0.05);而UC患者临床缓解组、轻度组、中度组以及重度组血清TLR4与MPO表达均呈线性正相关(P<0.05)。UC患者分组疾病活动程度相关分析显示,双歧杆菌、肠球菌以及真杆菌与疾病活动度呈正相关(P<0.05)。结论 UC患者肠道的菌群变化比较显著,同时TLR4、MPO表达差异较明显,提示双歧杆菌、肠球菌及真杆菌改变与UC发生发展密切相关。Objective To investigate the relationship between intestinal flora imbalance and serum toll⁃like receptor 4(TLR4),peroxidase(MPO)expression and disease activity in ulcerative colitis(UC)patients.Methods A total of 240 patients with UC from November 2020 to January 2023 were enrolled in this study.These patients with UC were divided into clinical remission group,mild group,moderate group and severe group,with 60 cases in each group.At the same time,60 healthy examiners were included as the control group.The intestinal bacteria,serum TLR4 and MPO levels were compared between the groups,and Pearson correlation analysis was performed.Results Compared with the control group,the contents of Bifidobacterium and Eubacterium decreased in the severe group(P<0.05),but the contents of Enterococcus did not change significantly.The contents of bifidobacteria and eubacteria in the clinical remission group were higher than those in the severe group(P<0.05),but still lower than those in the control group(P<0.05),and the contents of Enterococcus species were not significantly different compared with the control group(P>0.05).Compared with the control group,the expression of TLR4 and MPO in the mild group,moderate group and severe group increased gradually,and the expression level in the severe group increased significantly(P<0.05).Compared with the healthy control group,the expression levels of IgA,IgM and IgG in the severe group were decreased(P<0.05),and the expression levels of IgA,IgM and IgG in the clinical remission group were higher than those in the severe group(P<0.05),but still lower than those in the control group(P<0.05).Pearson correlation analysis showed that there was no relationship between TLR4 and MPO expression in the control group(P>0.05).However,there was a linear positive correlation of TLR4 with MPO expression in the clinical remission group,mild group,moderate group and severe group of UC patients(P<0.05).Correlation analysis of disease activity in UC patient showed that bifidobacteria,enterococci and eub
