草酸艾司西酞普兰联合奥扎格雷钠在老年急性脑梗死合并焦虑抑郁障碍患者中的应用

摘　　要：目的研究草酸艾司西酞普兰联合奥扎格雷钠在老年急性脑梗死(ACI)合并焦虑抑郁障碍患者中的应用价值。方法抽取2019年1月至2022年12月安阳市人民医院收治的老年ACI合并焦虑抑郁障碍患者78例,按照随机数字表法分为奥扎格雷钠组和奥扎格雷钠+草酸艾司西酞普兰组,每组39例。奥扎格雷钠组采用奥扎格雷钠治疗,奥扎格雷钠+草酸艾司西酞普兰组采用奥扎格雷钠联合草酸艾司西酞普兰治疗。比较两组临床疗效、神经肽Y(NPY)、P物质(SP)、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、日常生活活动能力[Barthel指数(BI)]、不良情绪[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)]及不良反应发生率。结果奥扎格雷钠+草酸艾司西酞普兰组总有效率(94.87%,37/39)高于奥扎格雷钠组(74.36%,29/39),P<0.05。治疗后,奥扎格雷钠+草酸艾司西酞普兰组NPY水平高于奥扎格雷钠组,SP水平低于奥扎格雷钠组(P<0.05);治疗后,奥扎格雷钠+草酸艾司西酞普兰组NIHSS评分低于奥扎格雷钠组,BI高于奥扎格雷钠组(P<0.05);治疗后,奥扎格雷钠+草酸艾司西酞普兰组HAMA、HAMD评分低于奥扎格雷钠组(P<0.05)。奥扎格雷钠+草酸艾司西酞普兰组不良反应发生率(20.51%,8/39)与奥扎格雷钠组(12.82%,5/39)比较差异未见统计学意义(P>0.05)。结论草酸艾司西酞普兰联合奥扎格雷钠治疗老年ACI合并焦虑抑郁障碍效果显著,能改善患者神经功能缺损症状及焦虑、抑郁情绪,提升其日常生活能力,且二者联合不会增加不良反应发生风险。Objective To investigate the clinical value of escitalopram oxalate combined with ozagrel sodium in elderly patients with acute cerebral infarction(ACI)complicated by anxiety and depression disorder.Methods A total of 78 elderly patients with ACI complicated by anxiety and depression disorder treated in Anyang People’s Hospital from January 2019 to December 2022 were selected,and they were divided into ozagrel sodium group and ozagrel sodium combined with escitalopram oxalate group according to random number table method,with 39 cases in each group.The ozagrel sodium group was treated with ozagrel sodium,and the ozagrel sodium combined with escitalopram oxalate group was treated with ozagrel sodium combined with escitalopram oxalate.The efficacy,neuropeptide Y(NPY),substance P(SP),neurological function assessed by American National Institute of Health stroke scale(NIHSS)score,daily living activity assessed by Barthel index(BI),bad motion assessed by Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD),and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the ozagrel sodium combined with escitalopram oxalate group(94.87%,37/39)was higher than that of the ozagrel sodium group(74.36%,29/39),P<0.05.After treatment,the NPY level of the ozagrel sodium combined with escitalopram oxalate group was higher than that of the ozagrel sodium group,and the SP level was lower than that of the ozagrel sodium group(P<0.05).After treatment,the NIHSS score in the ozagrel sodium combined with escitalopram oxalate group was lower than that in ozagrel sodium group,and BI was higher than that in the ozagrel sodium group(P<0.05).After treatment,the HAMA score and HAMD score in the ozagrel sodium combined with escitalopram oxalate group were lower than those in the ozagrel sodium group(P<0.05).There was no significant difference in the incidence of adverse reactions between the ozagrel sodium combined with escitalopram oxalate group(20.51%,8/39)and the ozagrel sodium g
