经纤维支气管镜确诊青年肺癌临床特点分析

摘　　要：目的探讨宁夏地区青年肺癌患者(年龄≤40岁)的一般临床特点、影像学及纤维支气管镜特点。方法收集172例年龄≤40岁的青年肺癌患者的病例资料并进行统计学分析。结果研究对象中男性82例(47.7%),女性90例(52.3%),以不吸烟女性居多,其中腺癌131例(76.2%),小细胞癌24例(14.0%)和鳞状细胞癌10例(5.8%)。青年肺腺癌影像学以周围型为主,常见结节、分叶及毛刺征;而小细胞肺癌及鳞癌主要表现为中央型,通常能观察到肿块、支气管截断等。青年肺癌支气管镜下主要以新生物及黏膜浸润、肥厚等直接征象(55.3%)为主。结论青年肺癌患者以不吸烟女性为多见,病理以腺癌为主。早期行胸部CT及纤维支气管镜检查对患者疾病早期诊断、提高患者的生存率和预后十分重要。Objective To explore the general clinical features,imaging and fibronchoscopic features of young patients with lung cancer(age≤40 years)in Ningxia.Methods Data of 172 lung cancer patients aged≤40 years old were collected and analyzed.Results There were 82 males(47.7%)and 90 females(52.3%).Most females were non-smoking.There were 131 cases of adenocarcinoma(76.2%),followed by small cell carcinoma with 24 cases(14.0%)and squamous cell carcinoma with 10 cases(5.8%).Adenocarcinoma in young people was mainly peripheral,with common nodules,lobulation,burrs signs.Small cell lung cancer and squamous cell carcinoma were predominantly central,with lumps and bronchial truncation.Under bronchoscopy,young patients with lung cancer were mainly characterized by direct signs such as neoplasm,mucosal infiltration and hypertrophy(55.3%).Conclusions Most of the lung cancer in young patients are more common in non-smoking women,and the pathology is mainly adenocarcinoma.Typical CT imaging and bronchoscopic findings can be seen.Early chest CT and fiberoptic bronchoscopy are very important for early diagnosis and improving the survival rate and prognosis.
