壳聚糖海绵在智齿拔牙术后的应用效果分析

摘　　要：目的探讨复杂下颌阻生智齿拔除后使用壳聚糖海绵减少术后并发症的临床效果。方法将2020年8月—2021年8月在电子科技大学医院口腔科行下颌中低位阻生第三磨牙拔除术的320例患者作为研究对象,根据随机数表法分组,壳聚糖海绵组108例,碘仿明胶海绵组104例,对照组108例,拔牙后分别给予壳聚糖海绵、碘仿明胶海绵填塞后缝合及无填塞直接缝合。观察拔牙后不同时间术后出血、张口受限、术区疼痛、伤口愈合不良及干槽症等并发症的发生情况。结果拔牙术后30 min,壳聚糖海绵组术后出血评分为(1.94±0.20)分、碘仿明胶海绵组术后出血评分为(2.21±0.25)分均明显低于对照组的(2.52±0.28)分,差异有统计学意义(F=150.93,P<0.05),术后3 d,3组出血评分比较,差异无统计学意义(P>0.05)。术后3、7 d,壳聚糖海绵组和碘仿明胶海绵组张口受限评分、愈合不良情况显著低于对照组,差异有统计学意义(P<0.05)。壳聚糖海绵组、碘仿明胶海绵组干槽症发生率均明显低于对照组,差异有统计学意义(P<0.05)。壳聚糖海绵组、碘仿明胶海绵组术后3、7 d视觉模拟评分法评分均低于对照组,差异有统计学意义(P<0.05)。结论壳聚糖海绵、碘仿明胶海绵填塞拔牙窝能减少术后出血、降低干槽症发生,且壳聚糖海绵的效果更佳。Objective To investigate the clinical effect of using chitosan sponge to reduce postoperative complications after extraction of complex mandibular retained genuine teeth.Methods 320 patients who underwent mandibular low and middle obstructed third molar extraction in the Department of Stomatology of University of Electronic Science and Technology Hospital from August 2020 to August 2021 were taken as subjects.They were grouped according to the randomized numerical table method,108 cases in the chitosan sponge group,104 cases in the iodoform gelatin sponge group,and 108 cases in the control group,and were given chitosan sponge,iodoform gelatin sponge fillings followed by sutures and direct sutures without fillings after extraction,respectively.Complications such as postoperative bleeding,restricted mouth opening,pain in the operative area,poor wound healing and dry socket were observed at different times after tooth extraction.Results 30 min after tooth extraction,the postoperative bleeding score of the chitosan sponge group was(1.94±0.20)points and the iodoform gelatin sponge group was(2.21±0.25)points were significantly lower than those of the control group(2.52±0.28)points,and the difference was statistically significant(F=150.93,P<0.05).There was no statistically significant difference in bleeding score among the three groups(P>0.05).The scores of restriction of mouth opening and poor healing in the chitosan sponge group and the iodoform gelatin sponge group were significantly lower than those in the control group at 3 d and 7 d after surgery,and the difference was statistically significant(P<0.05).The incidence of dry socket in chitosan sponge group and iodoform gelatin sponge group was significantly lower than that in control group,and the difference was statistically significant(P<0.05).The Visual Analogue Scale scores of chitosan sponge group and Iodoform gelatin sponge group at 3 d and 7 days after operation were lower than those of control group,and the difference was statistically significant(P<0.05)
