摘 要:目的 检测抑郁症患者和健康人群血清中嘌呤代谢物—次黄嘌呤、黄嘌呤、尿酸水平和黄嘌呤氧化酶活性的组间差异,探讨嘌呤代谢通路在抑郁症中的病理生理效应。方法 选择2022年10月—2023年3月本院心身医学科住院的抑郁症患者64例作为研究对象(以下简称患者组);另选同期体检中心检查人员或志愿者招募35名健康人员作为对照组。检测64例抑郁症患者和35名正常对照空腹血清样本中次黄嘌呤、黄嘌呤水平和黄嘌呤氧化酶活性。结果 抑郁症患者血清次黄嘌呤(P<0.001)、黄嘌呤(P<0.001)和尿酸(P=0.021)水平均明显低于对照,黄嘌呤氧化酶活性明显高于对照(P<0.001)。充分调整潜在混杂因素后logistic回归分析显示,与对照组相比,较低水平的血清次黄嘌呤(OR=0.50,95%CI 0.30~0.81)、黄嘌呤(OR=0.77,95%CI 0.66~0.91)、尿酸(OR=0.98,95%CI 0.97~0.99)和较高水平的黄嘌呤氧化酶(OR=1.03,95%CI 1.01~1.04)与抑郁症相关。结论 抑郁症患者存在嘌呤代谢失衡,嘌呤代谢紊乱与抑郁症发病机制相关,可能成为一类有效的诊断生物标记物。Objective To investigate the pathological effect of purine metabolic pathways in depression by detecting inter group differences in serum purine metabolites-hypoxanthine,xanthine,uric acid levels,and xanthine oxidase activity in patients with depression and healthy individuals.Methods Sixty-four patients with depression who were hospitalized in psychosomatic medicine department of the first affiliated hospital of Nanchang University from October 2022 to March 2023 were selected as the study objects(hereinafter referred to as the patient group);A total of 35 subjects(the individuals received physical examination or volunteers)were recruited as control group.The levels of hypoxanthine,xanthine and xanthine oxidase activity were detected in fasting serum samples from 64 patients with depression and 35 normal controls.Results The levels of serum hypoxanthine(P<0.001),xanthine(P<0.001),and uric acid(P=0.021)in patients with depression were significantly lower than those in healthy controls,and the activity of xanthine oxidase was significantly higher than that in healthy controls(P<0.001).After fully adjusting potential confounding factors,logistic regression analysis showed that compared with the healthy control group,low levels of serum hypoxanthine(OR=0.50,95%CI 0.30~0.81),xanthine(OR=0.77,95%CI 0.66~0.91),uric acid(OR=0.98,95%CI 0.97~0.99),and high levels of xanthine oxidase activity(OR=1.03,95%CI 1.01~1.04)were associated with depression.Conclusions Depression patients have purine metabolism imbalance,which confirms the correlation between purine metabolism disorder and depression,and may become an effective diagnostic biomarker.
