中期胃癌患者根治术后复发情况与术前LETM1、TIMP-1表达的关系研究

摘　　要：目的探讨中期胃癌患者根治术后复发情况与术前亮氨酸拉链EFhand结构域跨膜蛋白1(LETM1)、基质金属蛋白酶组织抑制剂(TIMP-1)的关系。方法回顾性分析行胃癌根治术的中期胃癌患者102例,术前行LETM1、TIMP-1检查,以术后3年内胃癌是否复发为评估标准,经ROC曲线分析术后3年,术前LETM1、TIMP-1及联合检测在评估胃癌根治术后复发状况中的预测效能(敏感度、特异度);比较生存时间≥3年及<3年患者术前LETM1、TIMP-1表达情况。结果术后3年内胃癌复发57例(55.88%),未复发45例(44.12%)。其中65例LETM1阳性患者中,共有48例(73.85%)复发,17例(26.15%)未复发;37例LETM1阴性患者中共有9例(24.32%)复发,28例未复发(75.68%);TIMP-1阳性共63例,有46例(73.02%)复发,17例未复发(26.98%);TIMP-1阴性共有39例,其中有11例(28.21%)复发,28例未复发(71.79%)。联合检测预测3年内复发71例(69.61%),无复发31例(30.39%)。LETM1高表达共54例,其中43例(79.63%)生存时间<3年,11例(20.37%)生存时间≥3年;LETM1低表达共48例,其中10例(20.83%)生存时间<3年,38例(79.17%)生存时间≥3年;TIMP-1阳性共有62例,其中46例(74.19%)生存时间<3年,16例(25.81%)生存时间≥3年;TIMP-1阴性共有40例,其中共有7例(17.50%)生存时间<3年,33例(82.50%)生存时间≥3年。结论中期胃癌根治术后是否复发与LETM1、TIMP-1表达关系密切,同时LETM1、TIMP-1检测亦可用于预测中期胃癌患者根治术后生存状况,且准确率较高,具备临床价值。Objective To explore the relationship between recurrence and preoperative leucine zipper EFhand containing transmembrane protein 1(LETM1)and tissue inhibitor of metalloproteinase-1(TIMP-1)in patients with middle-stage gastric cancer after radical gastrectomy.Methods 102 patients with middle-stage gastric cancer who underwent radical gastrectomy were retrospectively analyzed,and performed LETM1 examination and TIMP-1 examination before surgery.The presence or absence of recurrence of gastric cancer within 3 years after surgery was used as the evaluation standard.ROC curve was used to analyze the predictive efficiency(sensitivity,specificity)of preoperative LETM1,TIMP-1 and combined detection on evaluating the recurrence of gastric cancer after radical gastrectomy at 3 years after surgery.The expressions of preoperative LETM1 and TIMP-1 in patients with survival time≥3 years and<3 years were compared.Results According to statistics,there were 57 cases(55.88%)with recurrence of gastric cancer within 3 years after surgery and 45 cases(44.12%)without recurrence.Among the 65 patients with positive LETM1,there were 48 cases(73.85%)with recurrence and 17 cases(26.15%)without recurrence.There were 9 cases(24.32%)with recurrence and 28 cases(75.68%)without recurrence among the 37 patients with negative LETM1.63 patients were with positive TIMP-1,including 46 cases(73.02%)with recurrence and 17 cases(26.98%)without recurrence.39 patients were with negative TIMP-1,including 11 cases(28.21%)with recurrence and 28 cases(71.79%)without recurrence.Combined detection predicted 71 cases(69.61%)with recurrence and 31 cases(30.39%)without recurrence within 3 years.LETM1 was highly expressed in 54 patients,of which 43 cases(79.63%)had survival time<3 years and 11 cases(20.37%)had survival time≥3 years.LETM1 was lowly expressed in 48 patients,of which 10 cases(20.83%)had survival time<3 years and 38 cases(79.17%)had survival time≥3 years.62 patients were with positive TIMP-1,including 46 cases(74.19%)with survival time<3 years a
