超声造影检查在乳腺肿瘤诊断中的价值分析

摘　　要：目的:探究超声造影(CEUS)检查在乳腺肿瘤中的诊断价值。方法:选取2021年7月—2022年12月玉林市红十字会医院经病理检查确诊为乳腺肿瘤的患者78例为研究对象,以乳腺肿瘤组织病理检查结果为金标准,分析CEUS检查的诊断效果。结果:78例乳腺肿瘤组织病理检查结果中,恶性61例、良性17例;CEUS检查灵敏度87.69%、特异度60.23%、准确度84.62%。CEUS诊断乳腺肿瘤良、恶性病灶中,增强强度、造影剂分布形式、造影后边界、周边穿入或放射状增强、内部灌注缺损情况比较,差异有统计学意义(P<0.001)。恶性乳腺肿瘤达峰时间短于良性乳腺肿瘤,峰值强度高于良性乳腺肿瘤,明显减退时间长于良性乳腺肿瘤,差异有统计学意义(P<0.05)。结论:乳腺肿瘤的CEUS检查图像特征特异性、灵敏度较高,具有较高的诊断价值。Objective:To investigate the diagnostic value of contrast-enhanced ultrasound(CEUS)in diagnose of breast tumor.Methods:78 patients who were diagnosed as breast tumors by pathological examination in Yulin Red Cross Hospital from July 2021 to December 2022 were selected as the study subjects.The results of pathological test were treated as the gold standard and the diagnostic effect of CEUS was analyzed according to the gold standard.Results:among these 78 cases with positive path⁃ological results,61 cases were malignant and 17 cases were benign;the sensitivity of CEUS examination was 87.69%,the speci⁃ficity was 60.23%,and the accuracy was 84.62%.In the differential diagnosis of benign and malignant breast tumors under CEUS the enhancement intensity,contrast distribution form,post-contrast border,peripheral penetration or radial enhancement,and in⁃ternal perfusion defects were compared,and the difference was statistically significant(P<0.001).The peak time of malignant breast tumors was shorter than that of benign breast tumors,the peak intensity was higher than that of benign breast tumors,and the time of apparent regression was longer than that of benign breast tumors,and the difference was statistically signifi⁃cant(P<0.05).Conclusion:CEUS examination for breast tumors has high diagnostic value with high specificity and sensitivity of image features.
