高频电波刀手术与冷刀宫颈锥切术治疗宫颈上皮内瘤样病变的临床观察

摘　　要：目的:分析高频电波刀手术与冷刀宫颈锥切术对宫颈上皮内瘤样病变(CIN)患者的应用价值。方法:选取2021年10月—2023年10月赣州市宁都县妇幼保健院妇产科收治的CIN患者60例为研究对象,随机分为研究组和对照组,各30例。对照组接受冷刀宫颈锥切术治疗,研究组接受高频电波刀手术治疗。比较两组围术期指标、术后并发症、炎性指标、临床疗效。结果:研究组手术时间、术中出血量、切口愈合时间、术后并发症发生率均优于对照组,差异有统计学意义(P<0.05);术后3 d,两组白细胞介素-8、肿瘤坏死因子-α、C反应蛋白水平升高,研究组低于对照组,差异有统计学意义(P<0.05);研究组临床疗效总有效率高于对照组,差异有统计学意义(P=0.023)。结论:高频电波刀手术治疗CIN的手术时间较短,术中出血量较少,可减轻炎性反应,降低并发症发生率,有利于患者术后恢复,临床疗效确切。Objective:To analyze the application value of high-frequency electtosurgical surgery and cold knife cervical conization in patients with cervical intraepithelial neoplasia(CIN).Methods:Sixty patients with CIN admitted to the Department of Obstetrics and Gynecology of Ganzhou Ningdu County Maternal and Child Health Hospital from October 2021 to October 2023 were selected as the study subjects,and randomly divided into the study group and the control group,each with thirty cases.The control group was treated with cold knife cervical conization,and the study group was treated with high-frequency electtosurgical surgery.The perioperative indexes,postoperative complications,inflammatory indexes,and clinical efficacy were compared between the two groups.Results:The operation time,intraoperative bleeding,incision healing time,and the incidence of postoperative complications in the study group were better than those in the control group,and the difference was statistically significant(P<0.05);at 3 d postoperatively,the levels of interleukin-8,tumor necrosis factor-α,and C-reactive protein were increased in both groups,and the study group was lower than the control group,with a statistically significant difference(P<0.05);the total effective rate of clinical efficacy of the study group was higher than that of the control group,and the difference was statistically significant(P=0.023).Conclusion:High-frequency electtosurgical surgery for CIN has a shorter operation time,less intraoperative bleeding,reduces inflammatory reaction,reduces the incidence of complications,and is conducive to the patients'postoperative recovery,and the clinical efficacy is exact.
