摘  要:目的观察针刀干预对颈型颈椎病模型兔颈肌超声影像及肌肉组织内翅缘缺刻信号通路(notch signaling pathway,Notch)中的受体1(Notch1)及Delta样配体1(delta like ligand protein 1,Dll1)蛋白和基因表达的影响,探讨针刀干预对颈型颈椎病颈肌修复的效应机制。方法按随机数字表法将24只健康新西兰兔分为空白组、模型组和针刀组,每组各8只。除空白组外其余建立颈型颈椎病动物模型,持续8周。造模后通过颈椎侧位X线片验证是否造模成功。证实造模成功后实行针刀干预,每周1次,共4次。干预结束通过多普勒超声检查观察颈肌影像变化,采用蛋白质印迹法(Western blot)和实时荧光定量PCR法(quantitative real-time PCR,qRT-PCR)检测Notch1、Dll1蛋白和基因表达。结果造模后模型组和针刀组超声检查下可见局部肌肉回声增强、连续性中断,血流信号增强;干预后针刀组回声较均匀,血流信号减少。与空白组相比,模型组兔Notch1/Dll1蛋白和mRNA水平均明显升高(P<0.05);与模型组相比,针刀组Notch1/Dll1蛋白和mRNA水平明显降低(P<0.05)。结论针刀治疗能够明显下调Notch1/Dll1表达水平,促进肌肉修复,这可能是针刀治疗颈型颈椎病颈肌损伤的作用机制之一。Objective To observe the effect of acupotomology therapy on ultrasound image of cervical muscle and expression of Notch1/Dll1 protein and gene in rabbit model of cervical spondylosis of cervical type and explore the possible mechanism of acu⁃potomology intervention on the repair of cervical muscle injury.Methods Twenty-four healthy New Zealand rabbits were random⁃ly divided into three teams,including blank group,model group and acupotomology group,with eight rabbits in each group.Ex⁃cept the blank group,the other animals were used to establish the animal models of cervical spondylosis of cervical type by pro⁃longed flexed neck posture within eight weeks.The success of the model was verified by cervical X-ray lateral radiographs of each group.Acupotomy were respectively employed in acupotomy group after successfully modeling once a week,lasting for four times.Then at the next day after the end of the intervention,two rabbits in each group were randomly selected to observe the changes of neck muscles and related tissues by ultrasound.Western blot and qRT-PCR were used to detect the expressions of proteins and genes of Notch1/Dll1 in cervical muscles.Results In the model group and acupotomology group,the changes includ⁃ed increased muscle echo signal,interrupted continuity and increased blood flow signals.After intervention,the echo was more u⁃niform and the blood flow signal was less.Compared with those of the blank group,the Notch1/Dll1 protein and mRNA levels in the model group were significantly higher(P<0.05)and those in the acupotomology group were significantly lower(P<0.05).Conclusion Acupotomy can significantly reduce Notch1/Dll1 level and promote muscle repair,which may be one of the mecha⁃nisms of acupotomy in the treatment of cervical muscle injury in cervical spondylosis of cervical type possibly.
