摘　　要：目的:探讨乳腺癌区域淋巴结切除与术后肿瘤局部复发的关系。方法:收集2011年8月1日至2018年12月31日于石河子大学第一附属医院胃肠乳腺外科行手术治疗的pT_(1-2)N_(0)M_(0)期女性乳腺癌患者232例。所有患者按区域淋巴结切除数目进行分组,对乳腺癌术后局部复发的影响因素进行单因素和多因素分析。结果:单因素分析显示pT_(1-2)N_(0)M_(0)期乳腺癌患者术后局部复发与区域淋巴结切除、PR、内分泌治疗、脉管神经癌侵有关。多因素分析显示,区域淋巴结切除、脉管神经癌侵是pT_(1-2)N_(0)M_(0)期乳腺癌患者术后局部复发的独立危险因素。结论:pT_(1-2)N_(0)M_(0)期乳腺癌患者术后局部复发与区域淋巴结切除密切相关,但其复发的生物学机制尚待进一步研究。Objective:To investigate the relationship between regional lymphadenectomy and postoperative local recurrence of breast cancer.Methods:A total of 232 female patients with stage pT_(1-2)N_(0)M_(0)breast cancer who underwent surgery in the Department of Gastrointestinal and Breast surgery,the first affiliated Hospital of Shihezi University from August 1,2011 to December 31,2018 were collected.All patients were divided into groups according to the number of regional lymph node resection,and the influencing factors of postoperative local recurrence of breast cancer were analyzed by univariate and multivariate analysis.Results:Univariate analysis showed that postoperative local recurrence in patients with stage pT_(1-2)N_(0)M_(0)breast cancer was related to regional lymphadenectomy,PR,endocrine therapy and vascular nerve cancer invasion.Multivariate analysis showed that regional lymphadenectomy and vascular nerve cancer invasion were independent risk factors for postoperative local recurrence in patients with stage pT_(1-2)N_(0)M_(0)breast cancer.Conclusions:Postoperative local recurrence of stage pT1-2N0M0 breast cancer is closely related to regional lymphadenectomy,but the biological mechanism of recurrence remains to be further studied.
