摘 要:目的探讨磁共振成像(MRI)联合罗马指数对卵巢肿瘤的诊断价值,以期为临床完善治疗方案提供依据。方法选取2020年3月至2023年2月平煤神马集团总医院收治的卵巢肿瘤者107例,以术后病理结果作为金标准,比较不同性质卵巢肿瘤者的K_(ep)、K_(trans)、罗马指数,利用Pearson分析K_(ep)、K_(trans)、罗马指数与恶性卵巢肿瘤患者病理分期的相关性,利用受试者工作特征曲线(ROC)分析MRI联合罗马指数对卵巢肿瘤的诊断价值。结果术后病理结果为恶性肿瘤65例、良性肿瘤42例;良性卵巢肿瘤患者K_(ep)、K_(trans)、罗马指数分别为(0.10±0.02)min^(-1)、(0.23±0.04)mm^(-1)、(17.96±4.68)%,明显低于恶性卵巢肿瘤患者的(0.54±0.10)min^(-1)、(0.36±0.08)mm^(-1)、(62.24±17.40)%,差异均具有统计学意义(P<0.05);经Pearson分析结果显示,Kep、Ktrans、罗马指数与恶性卵巢肿瘤患者病理分期均呈正相关(r=0.638、0.642、0.719,P<0.05);经ROC分析结果显示,血清K_(ep)、K_(trans)、罗马指数联合诊断恶性肿瘤的曲线下面积(AUC)均大于单一诊断,差异均具有统计学意义(P<0.05)。结论MRI联合罗马指数应用于卵巢肿瘤患者的诊断,可发挥良好诊断效能,并能反映恶性肿瘤病情程度,为临床治疗方案制定提供数据支持。Objective To analyze the diagnostic value of magnetic resonance imaging(MRI)combined with Rome index in ovarian tumors,in order to provide evidence for clinical improvement of treatment.Methods A total of 107 patients with ovarian tumor in General Hospital of Pingmei Shenma Group from March 2020 to February 2023 were selected.Postoperative pathological results were used as the gold standard,and Kep,Ktrans,and Roman indexes of patients with different types of ovarian tumors were compared.Pearson was used to analyze the correlation between Kep,Ktrans,Ro-man index and pathological stage of patients with malignant ovarian tumors.The receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of MRI combined with Roman index for ovarian tumors.Results Postoperative pathological results showed 65 cases of malignant tumors and 42 cases of benign tumors.The K_(ep),K_(trans),and Roman index of patients with benign ovarian tumors were(0.10±0.02)min^(-1),(0.23±0.04)mm^(-1),and(17.96±4.68)%,respectively,which were significantly lower than(0.54±0.10)min^(-1),(0.36±0.08)mm^(-1),and(62.24±17.40)%of patients with malignant ovarian tumors(P<0.05).The results of Pearson analysis showed that Kep,Ktrans,and Roman index were positively correlated with the pathological stage of patients with malignant ovarian tumors(r=0.638,0.642,0.719,P<0.05).The results of ROC analysis showed that the area under the curve(AUC)of serum K_(ep),K_(trans) and Roman index in the diagnosis of malignant tumors was greater than that of single diagnosis,and the difference was statistically significant(P<0.05).Conclusion MRI combined with Rome index in the diagnosis of ovarian tumor patients has a good diagnostic efficacy and can reflect the degree of malignant tumor disease,which provides data support for the formulation of clinical treatment plan.
