芒硝腹部外敷治疗轻症型胆源性胰腺炎32例

摘　　要：目的 观察芒硝腹部外敷治疗轻症型胆源性胰腺炎病人的疗效。方法 选取2017年11月至2021年12月于遂宁市中医院肝胆外科行腹腔镜胆囊切除的轻症型胆源性胰腺炎病人62例。常规治疗30例(对照组),常规治疗加用芒硝腹部外敷组32例(观察组)。比较两组病人白细胞(WBC)、C反应蛋白(CRP)、术区粘连、术前等待时间、手术时间、住院时间等指标。结果观察组在入院第5天WBC、CRP改善明显优于对照组,差异有统计学意义(P<0.01);观察组与对照组粘连评分[(1.06±0.56)分、(1.97±0.56)分],术前等待时间[(5.69±0.74)d、(6.53±0.73)d],手术时间[(38.84±5.77)min、(49.77±8.60)min],住院时间[(8.97±1.06)d、(9.80±1.13)d]比较,均差异有统计学意义(P<0.05)。结论 芒硝腹部外敷可降低轻症型胆源性胰腺炎病人炎症反应、减轻术区粘连、缩短等待手术、手术及住院时间。Objective To investigate the effect of abdominal application of mirabilite on patients with mild acute biliary pancreatitis(MABP).Methods Sixty-two patients with MABP,who underwent laparoscopic cholecystectomyin theDepartment of Hepatobiliary Surgeryof Suining Hospital of Traditional Chinese Medicinefrom November 2017 to December 2021,were chosen forthe study.The con-trol group(n=30)received conventional treatment,while the study group(n=32)received conventional treatment plus external applica-tion of mirabilite to the abdomen.White blood cell(WBC),C-reactive protein(CRP),surgical site adhesion,preoperative waiting time,operation timeand length ofhospitalstay were compared between the two groups.Results The study group had a significantly better de-crease in WBC and CRP on the 5th day of admission than the control group with statistically significant difference(P<0.01).Differences were found to be significant between the two groups in score of adhesion[(1.06±0.56)points vs.(1.97±0.56)points],preoperative waiting time[(5.69±0.74)d vs.(6.53±0.73)d],operation time[(38.84±5.77)min vs.(49.77±8.60)min],length of hospital stay[(8.97±1.06)d vs.(9.80±1.13)d](P<0.05).Conclusion Abdominal external application of mirabilite in MABP patients can decrease inflammation reac-tion,reduce surgical site adhesion,shorten preoperative waiting time,operation time and length of hospital stay.
