不同年龄阶段脑卒中病人生活质量现状分析

摘　　要：目的基于国际功能、残疾和健康分类(ICF)理论框架探讨不同年龄阶段脑卒中病人生活质量现状及其影响因素。方法选取脑卒中病人238例,分为中青年组(18~59岁)和老年组(≥60岁),经问卷调查法获取相关信息。结果中青年脑卒中病人生活质量总分高于老年脑卒中病人(t=2.286,P<0.05)。多元逐步线性回归分析结果显示,中青年和老年脑卒中病人生活质量的共同影响因素包括抑郁、活动受限、社会参与受限和神经功能缺损严重程度(t=-5.128~-2.883,P<0.05),影响中青年和老年脑卒中病人生活质量的因素还分别包括环境因素和合并慢性疾病数量(t=-3.345、-2.425,P<0.05)。结论中青年脑卒中病人的生活质量较老年脑卒中病人高。中青年和老年脑卒中病人生活质量的影响因素既有共性,又有特异性,医务人员应采取个性化的干预措施。Objective To investigate the status quo of and factors influencing thequality of life ofpatients with stroke of different agesbased on the theoretical framework of the International Classification of Functioning,Disability,and Health.Me-thods A total of 238 patients with strokeweredivided intoyoung and middle-aged group(18-59 years old)and elderly group(≥60 years old).Relevant information was obtained througha questionnaire survey.Results The total score of quality of life of the young and middle-aged patients was significantly higher than that of the elderly patients(t=2.286,P<0.05).The multivariable stepwise regression analysis showed that the common factors influencing the quality of life of young and middle-aged and elderly patients with stroke were depression,activity limitation,social participation limitation,and the degree of neurological function impairment(t=-5.128 to-2.883,P<0.05);in addition,environmental factors could influence the quality of life of young and middle-aged patients,and the number of chronic diseases could affect the quality of life in elderly patients(t=-3.345,-2.425,P<0.05).Conclusion Young and middle-aged patientswith stroke have better quality of life than elderly patients.The commonality and specificity of factors affecting quality of life in young and middle-aged and old patients with stroke suggest that cliniciansshould take individualized intervention measures.
