香砂养胃汤加减联合替普瑞酮治疗慢性浅表性胃炎临床研究

摘　　要：目的:观察香砂养胃汤加减联合替普瑞酮治疗慢性浅表性胃炎(CSG)脾胃虚寒证的临床疗效。方法:选取108例CSG脾胃虚寒证患者,按照随机数字表法分成对照组和研究组各54例。对照组给予替普瑞酮治疗,合并幽门螺杆菌(Hp)感染者加用三联疗法治疗,研究组在对照组基础上给予香砂养胃汤加减治疗,2组均治疗4周。比较2组临床疗效、中医证候积分及炎症指标[白细胞介素-8 (IL-8)、C-反应蛋白(CRP)、单核细胞趋化蛋白因子-1 (MCP-1)]水平,记录不良反应。结果:治疗后,研究组总有效率92.59%,高于对照组77.78%(P<0.05)。2组胃脘疼痛、喜温喜按、食后胀满、纳呆少食、大便稀溏、神疲乏力积分均较治疗前降低(P<0.05),研究组上述6项中医证候积分均低于对照组(P<0.05)。2组血清IL-8、CRP、MCP-1水平均较治疗前降低(P<0.05),研究组血清IL-8、CRP、MCP-1水平均低于对照组(P<0.05)。治疗期间,2组均未见明显不良反应发生。结论:香砂养胃汤加减联合替普瑞酮治疗CSG脾胃虚寒证能提高临床疗效,有效缓解患者的临床症状,抑制炎症反应,且安全性好。Objective:To observe the clinical effect of the therapy of modified Xiangsha Yangwei Decoction combined with Teprenone on chronic superficial gastritis(CSG)of spleen-stomach deficiency cold syndrome.Methods:A total of 108 cases of CSG patients with spleen-stomach deficiency cold syndrome were selected and divided into the control group and the study group according to the random number table method,with 54 cases in each group.The control group was given Teprenone for treatment,of which the patients complicated with helicobacter pylori(Hp)infection were additionally treated with triple therapy,and the study group was additionally treated with modified Xiangsha Yangwei Decoction based on the treatment of the control group.Both groups were treated for 4 weeks.The clinical effects,traditional Chinese medicine(TCM)syndrome scores and levels of inflammatory indicators including interleukin-8(IL-8),C-reactive protein(CRP)and monocyte chemotactic protein-1(MCP-1)were compared between the two groups.The incidence of adverse reactions were recorded.Results:After treatment,the total effective rate was 92.59%in the study group,higher than that of 77.78%in the control group(P<0.05).The scores of epigastric pain,desire for warmth and pressing,distention and fullness after eating,torpid intake and poor appetite,sloppy stool and mental fatigue and lack of strength in both groups were decreased when compared with those before treatment(P<0.05),and the above six TCM syndrome scores in the study group were lower than those in the control group(P<0.05).The levels of serum IL-8,CRP and MCP-1 in both groups were decreased when compared with those before treatment(P<0.05),and the above three levels in the study group were lower than those in the control group(P<0.05).During the treatment period,no significant adverse reactions were observed in both groups.Conclusion:The therapy of modified Xiangsha Yangwei Decoction combined with Teprenone can enhance the clinical effect on CSG patients with spleen-stomach deficiency cold syndrome,whic
