黏液型脂肪肉瘤的MRI影像学特征与预后的相关性分析

摘　　要：目的探讨黏液型脂肪肉瘤(MLS)的MRI影像学特征,分析肿瘤成分与预后的相关性。方法选取并分析13例经病理证实的MLS患者的MRI影像学特征及其与预后的相关性,并计算其敏感性和特异性;根据术后随访观察有无复发或转移,将MLS患者分为预后良好组与预后不良组。结果预后良好组肿瘤的非脂肪/非黏液成分比例平均为(21.67±14.72)%,预后不良组平均为(60.14±12.08)%,敏感性83.33%,特异性85.71%,P<0.001;预后良好组肿瘤的黏液成分比例平均为(69.50±17.13)%,预后不良组平均为(35.00±16.07)%,敏感性100%,特异性14.29%,P=0.003;预后良好组肿瘤的ADC低信号比例平均为(2.33±2.73)%,预后不良组平均为(23.86±19.64)%,敏感性100%,特异性85.71%,P=0.027,差异均有统计学意义(P均<0.05)。结论MLS的MRI表现具有一定的特征性,如非脂肪/非黏液成分中的弥散受限区可能是圆细胞成分密集区,分析肿瘤的成分对术前穿刺活检具有指导意义,并对预后有一定的预测价值。Objective To investigate the MR imaging features of Myxoid liposarcoma(MLS)and to analyze the correlation between tumor components and prognosis.Methods We retrospectively analyzed the MR imaging features of 13 MLS patients confirmed pathologically and their correlation with prognosis,and then,calculated their sensitivity and specificity.The MLS pa⁃tients were divided into good prognosis group or poor prognosis group according to whether there was recurrence or metastasis in postoperative follow-up.Results The average ratio of non-fatty/non-Myxoid components in tumors with good prognosis was(21.67±14.72)%,while,that with poor prognosis was(60.14±12.08)%,with sensitivity of 83.33%and specificity of 85.71%,P<0.001.The proportion of Myxoid components in tumors with good prognosis was(69.50±17.13)%on average,while,that in tumors with poor prognosis was(35.00±16.07)%on average,with sensitivity of 100%and specificity of 14.28%,P=0.003.The average ADC low signal ratio of tumors with good prognosis was(2.33±2.73)%,and that with poor prognosis was(23.86±19.64)%,with sensitivity of 100%and specificity of 85.71%,P=0.027.Conclusion The MR imaging of MLS has certain characteristics,for example,the diffusion restricted area of non-fatty/non-Myxoid components may be the area with dense round cell components.The analysis of tumor components has guiding significance for preoperative puncture biopsy and certain predictive value for prognosis.
