摘  要:目的探讨骨质疏松症患者膳食纤维摄入量对肠道菌群组成和结构变化的影响。方法选取2022年9月至12月于广西医科大学第一附属医院72例初诊骨质疏松症的患者。通过半定量食物频率调查问卷和中国营养食物成分表第六版获取膳食纤维摄入量信息。根据膳食纤维中位摄入量分为:高膳食纤维组(≥11.92 g/d,n=36)及低膳食纤维组(<11.92 g/d,n=36)。使用Illumina MiSeq测序平台进行16S rRNA基因高通量测序,运用R语言和Qiime程序对测序结果进行物种注释和菌群结构差异分析。结果两组样本Beta多样性差异有统计学意义(P=0.007)。LefSe分析结果显示,变形菌门、γ-变形菌纲、肠杆菌目、肠杆菌科是低膳食纤维组相对丰度显著增加的菌群,Agathobacter和缠结真杆菌是高膳食纤维组相对丰度显著增加的菌群。广义线性回归模型结果显示,与膳食纤维摄入水平正相关的肠道菌群包括厚壁菌门、疣微菌门、梭杆菌门、疣微菌纲、疣微菌目、梭杆菌目、阿克曼氏菌科、乳酸乳球菌、Agathobacter、阿克曼氏菌属、脆弱类杆菌和梭杆菌属;与膳食纤维摄入水平负相关的属或种水平的肠道菌群包括变形菌门、γ-变形菌纲、乳杆菌目、鞘氨醇杆菌目、气球菌科和大肠埃希氏菌。肠道菌群与血清骨代谢标志物的相关性分析显示β-CTx与疣微菌门、疣微菌纲、疣微菌目、阿克曼氏菌科和阿克曼氏菌属呈显著负相关(P<0.05),与大肠杆菌呈显著正相关(P<0.05)。PINP与埃氏拟杆菌呈显著正相关(P=0.030)。结论较高的膳食纤维摄入量可能通过改变肠道菌群的结构组成从而对骨质疏松症的疾病进程产生积极影响,适量补充膳食纤维可能是未来营养干预防治骨质疏松的潜在途经之一。Objective To investigate the relationship between dietary fiber intake and intestinal flora composition and structure in osteoporosis patients.Methods A total of 72 patients with osteoporosis were selected from the First Affiliated Hospital of Guangxi Medical University from September to December 2022.The information of dietary fiber intake was obtained through the semi-quantitative food frequency questionnaire and the sixth edition of China Nutrition Food Composition Table.According to the median dietary fiber intake,they were divided into high dietary fiber group(≥11.92 g/d,n=36)and low dietary fiber group(<11.92 g/d,n=36).Illumina MiSeq sequencing platform was used to conduct high-throughput sequencing of 16S rRNA gene,and R language and Qiime program were used to annotate the sequencing results and analyze the differences in bacterial community structure.Results The difference of Beta diversity between the two groups was statistically significant(P=0.007).Lef Seanalysisresults showed that Proteobacteria,gamma-Proteobacteria,Enterobacteriaceae and Enterobacteriaceae family were the bacteria groups with significant increases in relative abundance in the low dietary fiber group.The relative abundance of Agathobacter and Eubacterium nodatum increased significantly in the high dietary fiber group.The results of generalized linear regression model showed that the intestinal flora positively correlated with dietary fiber intake level included Phylum Firmicutes,Verrucomicrobia,Fusobacteria,Verrucomicrobiales,Verrucomicrobiaceae,Fusobacteriaceae,Akkermansiaceae,Lactococcus lactis,Agathobacter,Akkermansia,b.fragilis,Fusobacterium.Genera or species levels of intestinal flora negatively associated with dietary fiber intake included Proteobacteria,Gammaproteobacteria,Lactobacillales,Sphingomonadales,Aerococcaceae,and Escherichia.Correlation analysis between intestinal flora and serum markers of bone metabolism showed thatβCTx was significantly negatively correlated withVerrucomicrobia,Verrucomicrobiales,Verrucomicrobia
