摘  要:目的:研究肿瘤坏死因子相关凋亡诱导配体(tumor-necrosis-factor-related apoptosis-inducing ligand, TRAIL蛋白在肝细胞癌(hepatocellular carcinoma, HCC)组织中的表达水平,分析其与临床病理特征及预后的相关性,并探讨TRAIL对HCC细胞凋亡、增殖及耐药的影响。方法 :利用免疫组织化学法检测186例HCC患者中HCC组织及癌旁肝组织TRAIL、Caspase-3、NF-κB、P-糖蛋白(P-glycoprotein, P-gP)及Ki-67的表达,分析TRAIL与临床病理特征及预后的关系,并探讨TRAIL与HCC生物学行为(增殖、凋亡、耐药)的相关性。结果:HCC组织中TRAIL表达明显低于癌旁肝组织(P<0.01)。HCC中Caspase-3、NF-κB、P-gP的阳性表达率分别为38.2%、84.4%、66.1%,Ki-67在HCC中的高表达率为75.8%,低表达率为24.2%。Spearman相关性分析显示TRAIL与Caspase-3的表达呈正相关(P<0.01),与NF-κB、P-gP、Ki-67的表达均呈负相关(均P<0.05)。Kaplan-Meier生存分析显示,TRAIL阳性表达患者的平均生存时间显著长于阴性表达者(P<0.01)。单因素和Cox模型多因素分析结果显示TRAIL低表达与术后复发是HCC预后的独立影响因素(P<0.05)。结论:TRAIL在HCC中低表达,并与脉管癌栓、肿瘤病理分级、术后复发有关,提示TRAIL低表达是HCC恶性进展中的关键性分子事件。检测TRAIL表达在评价HCC分级、浸润、复发及预后中有一定的价值。Objective:To investigate the expression levels of tumor-necrosis-factor-related apoptosis-inducing ligand(TRAIL)protein in hepatocellular carcinoma(HCC),and analyze the correlation of TRAIL with clinicopathological characteristics and prognosis of HCC patients.And to investigate the effect of TRAIL on the apoptosis,proliferation and drug resistance of HCC cells.Methods:The expression of TRAIL,Caspase-3,NF-κB,P-glycoprotein(P-gP)and Ki-67 in 186 samples of HCC,paracancerous liver tissues were detected by immunohistochemistry.To evaluate the relationship between expressions of TRAIL with the clinical pathological factors and prognosis.And to explore the correlation between TRAIL and the biological behaviors(proliferation,apoptosis,and drug resistance)of HCC.Results:The positive expression of TRAIL was significantly decreased in HCC compared with the paracancerous liver tissues(P<0.01).The positive expression rates of Caspase-3,NF-κB and P-gP were 38.2%,84.4%,66.1%.Ki-67 was found to be expressed in HCC tissues,including 75.8%high positive and 24.2%weak positive.Spearman correlation analysis showed that there was positive correlation between the TRAIL and Caspase-3 protein expression(P<0.01).Whereas TRAIL expression was negatively correlated to the NF-κB,P-gP and Ki-67(all P<0.05)in HCC.Kaplan-Meier survival analysis showed that the average survival time of patients with positive expression of TRAIL was significantly longer than that of patients with negative expression of TRAIL(P<0.01).Univariate and Cox proportional hazards model analysis showed that the low expression levels of TRAIL and recurrence after operation were the independent influencing factors of prognosis of the patients with HCC(P<0.05).Conclusion:TRAIL was low expressed in HCC and was related to vascular tumor thrombus,tumor differentiation and recurrence after operation,which indicated that the low expression of TRAIL was a key molecular event in the malignant progression of HCC.Detecting the expression of TRAIL has certain value in the evaluati
