IOLMaster 700与CASIA2角膜曲率的分析比较

摘　　要：目的比较IOLMaster 700与CASIA2在屈光不正患者中角膜曲率的相关性,一致性和差异性。方法回顾性病例研究。纳入四川大学华西医院眼科83例屈光不正患者,共166只眼,在30 min内分别进行IOLMaster 700与CASIA2测量,并对各参数进行分析,包括模拟角膜陡峭轴曲率(SimKs),模拟角膜平坦轴曲率(SimKf),模拟角膜散光(C,J0和J45),角膜后表面陡峭轴曲率(PKs),角膜后表面平坦轴曲率(PKf),角膜后表面散光(C_(P),J0_(P)和J4_(P))以及全角膜陡峭轴曲率(TKs),全角膜平坦轴曲率(TKf),全角膜散光(C_(T),J0_(T)和J45_(T))。结果IOLMaster 700与CASIA2所测的模拟角膜曲率simKf及散光,后表面曲率及散光,以及全角膜曲率及散光C_(T)、J0_(T)差异均有统计学意义(P<0.05);两种仪器模拟角膜曲率,全角膜曲率以及角膜后表面测量值均有正相关性(r>0,P<0.05);Bland⁃Altman分析得出模拟角膜散光J0,J45,角膜后表面曲率及散光,以及全角膜散光C_(T)及J45_(T)95%一致性界限(95%LoA)范围较窄,一致性较好。结论IOLMaster 700与CASIA2在测量屈光不正患者中角膜后表面具有较好的一致性,在临床运用中可以相互转换,但模拟角膜曲率,全角膜曲率在相互替代使用时应根据临床做出选择。Objective To evaluate the agreement,correlation and difference of corneal curvature and astigmatism in patients with refractive error between intraocular lens IOLMaster 700 and CASIA 2.Methods This retrospective study involved 83 patients with refractive error in West China Hospital.They were tested with IOLMaster 700 and CASIA2 in 30 minutes.The simulated steep keratometry(Ks),the simulated flat keratometry(Kf),the astigmatism(C,J0 and J45)of simulated keratometry,the posterior keratometry(PKs and PKf),the posterior corneal astigmatism(C_(P),J0_(P) and J45_(P)),the total keratometry(TKs and TKf)and the total corneal astigmatism(C_(T),J0_(T) and J45_(T))were recorded.Results The simKf,C,J0 and J45,PKs,PKf,C_(P),J0_(P) and J45_(P),TKs,TKf,C_(T) and J0_(T) obtained by IOLMaster 700 and CASIA2 showed significant difference(P<0.05).All the parameters of simulated keratometry,the total keratometry and posterior keratometry showed positive correlation by Pearson correlation coefficient(r>0,P<0.05).The J0 and J45 of simulated keratometry,posterior keratometry,C_(T) and J45_(T) of total keratometry showed good agreement by Bland⁃Airman。Conclu⁃sions There are good agreement on the posterior keratometry on IOLMaster 700 and CASIA2,and they can provide refer⁃ences to another.Otherwise,simulated keratometry and total keratometry should choose the best to determine.
