老年患者颌面部软组织Ⅱ类手术切口愈合情况及影响因素分析

摘　　要：目的探讨老年患者颌面部软组织Ⅱ类手术切口愈合情况及其影响因素。方法回顾性分析2021年1月—2023年1月濮阳市人民医院口腔科140例行手术治疗的口腔颌面部软组织损伤老年患者的临床资料,均为Ⅱ类手术切口。根据术后2周切口愈合情况分为愈合良好组、愈合不良组。比较两组患者一般资料,采用多因素Logistic回归分析法分析术后切口愈合的影响因素。结果愈合良好组与愈合不良组患者年龄、ASA分级、伤口深度、切口长度、手术时间、住院时间、吸烟史、合并糖尿病、HBV感染及血清白蛋白、纤维蛋白原水平比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,吸烟史、合并糖尿病、HBV感染、白蛋白降低、纤维蛋白原升高是切口愈合不良的独立危险因素(P<0.05)。结论口腔颌面部软组织损伤Ⅱ类手术切口老年患者术后切口愈合影响因素较多,包括年龄、合并糖尿病、HBV感染、白蛋白降低、纤维蛋白原升高等。Objective To investigate the wound healing of maxillofacial soft tissue type II surgery in elderly patients and its influencing factors.Methods The clinical data of 140 elderly patients with oral and maxillofacial soft tissue injury underwent surgical treatment in Stomatology Department of Puyang People’s Hospital from January 2021 to January 2023 were retrospectively analyzed,all of which were Class II surgical incisions.According to the wound healing condition 2 weeks after surgery,the group was divided into good healing group and poor healing group.The general data of the two groups were compared,and the influencing factors of postoperative incision healing were analyzed by multivariate Logistic regression analysis.Results There were statistically significant differences in age,ASA grading,wound depth,incision length,operation time,hospital stay,smoking history,diabetes mellitus,HBV infection and serum albumin and fibrinogen levels between good healing group and poor healing group(P<0.05).Multiple Logistic regression analysis showed that smoking history,diabetes mellitus,HBV infection,Albumin decreased and fibrinogen increased were independent risk factors for poor wound healing(P<0.05).Conclusion There are many factors affecting wound healing in elderly patients with type II incision for oral and maxillofacial soft tissue There are many factors influencing wound healing in elderly patients with oral and maxillofacial soft tissue injury,including age,diabetes mellitus,HBV infection,decreased albumin,elevated fibrinogen,etc.
