摘 要:目的:分析高频彩色多普勒血流成像(color Doppler flow imaging, CDFI)联合声脉冲辐射力弹性成像(acoustic radiation force impulse, ARFI)技术在乳腺肿瘤鉴别中的应用价值。方法:于我院伦理委员会批准后前瞻性收集2017年2月至2021年2月146例连续性门诊或住院的女性乳腺占位性病变患者,共计153个病灶,以病理组织学检查为“金标准”,采用高频CDFI、ARFI技术对女性乳腺占位性病变患者进行检测,分析诊断价值。结果:良性、恶性乳腺肿瘤在高频CDFI超声征象血流级别、Vmax、RI 3方面存在统计学差异(P<0.05)。高频CDFI、ARFI技术、高频CDFI联合ARFI技术对乳腺肿瘤诊断的灵敏度分别为71.09%、81.25%、91.41%,特异度分别为84.00%、64.00%、56.00%。ROC曲线分析高频CDFI、ARFI技术、高频CDFI联合ARFI技术对乳腺肿瘤诊断的灵敏度分别为76.39%、81.27%、85.46%,特异度分别为83.49%、77.15%、74.62%。结论:高频CDFI联合ARFI技术在乳腺肿瘤鉴别中应用价值较高,可提高鉴别诊断的灵敏度、准确度。Objective:Analyze the application value of high frequency color Doppler flow imaging(CDFI)combined with acoustic radiation force elastography(ARFI)in the differential diagnosis of breast tumors.Methods:After the approval of the ethics committee of our hospital,146 cases of female breast space occupying lesions in our hospital from February 2017 to February 2021 were included in the study,with a total of 153 lesions.Taking histopathological examination as the"gold standard",high-frequency CDFI and ARFI technology were used to detect female breast space occupying lesions,and the diagnostic value was analyzed.Results:The blood flow grade,Vmax and RI of high-frequency CDFI signs of benign and malignant breast tumors were significantly higher than those of benign and malignant breast tumors(P<0.05).The sensitivity of high frequency CDFI,ARFI and high frequency CDFI combined with ARFI in the diagnosis of breast cancer were 71.09%,81.25%and 91.41%respectively,and the specificity were 84.00%,64.00%and 56.00%respectively.R0C curve analysis showed that the sensitivity of high frequency CDFI,ARFI and high frequency CDFI combined with ARFI in the diagnosis of breast cancer were 76.39%,81.27%and 85.46%respectively,and the specificity were 83.49%,77.15%and 74.62%respectively.Conclusion:High frequency CDFI combined with ARFI technology has high application value in the differential diagnosis of breast tumors,which can improve the sensitivity and accuracy of differential diagnosis.
