摘　　要：目的探讨CD36在肝细胞癌组织中的表达情况及其临床意义。方法通过实时荧光定量聚合酶链反应技术分析来源于我院69例肝细胞癌患者的癌组织及其癌旁组织样本中CD36的表达情况,并收集病例资料及随访信息,研究其与肝细胞癌患者临床病理特征及预后的相关性。结果在癌组织中,CD36的表达量为(0.762±0.077),与癌旁组织中的表达量(0.464±0.051)相比,明显处于较高水平(P≤0.001);并与患者血清AFP水平(P=0.011)、CNLC分期(P=0.017)、病理分化等级(P=0.006)及是否合并有微血管侵犯(P=0.007)明显相关,但不受年龄、性别、肿瘤数量、肿瘤直径及乙型肝炎病毒感染等因素影响;CD36低表达组及高表达组的1、3和5年总生存率分别为83%、67.5%及40.2%,75%、35.9%及20.5%(χ2=4.74,P=0.029);经多因素Cox回归分析发现,CD36高表达量、血清AFP≥400μg/L、肿瘤分化程度高及合并微血管侵犯是影响肝细胞癌患者预后生存的独立危险因素。结论CD36在肝细胞癌组织中呈表达上调趋势,且与血清AFP水平、病理分化等级、CNLC分期、是否合并微血管侵犯及预后生存密切相关,提示CD36在肝细胞癌的发生和预后中具有重要作用。Objective To analyze the expression and clinical value of CD36 in hepatocellular carcinoma(HCC).Methods The expression of CD36 in cancer tissues and adjacent tissues from 69 patients with HCC in our hospital was detected by real-time fluorescence quantitative polymerase chain reaction.The case data and follow-up information were collected to study its correlation with clinicopathological characteristics and prognosis of patients with HCC.Results The relative expression level of CD36 in HCC was(0.762±0.077),which was significantly higher than that in adjacent tissues(0.464±0.051)(P≤0.001).It was significantly correlated with the expression of AFP in serum(P=0.011),CNLC stage(P=0.017),pathological differentiation grade(P=0.006)and microvascular invasion(P=0.007),but not affected by age,gender,number of tumors,tumor diameter and hepatitis B virus status.The total survival rates of the 1,3 and 5 years of CD36 low expression group and high expression group were 83%,67.5%and 40.2%,75%,35.9%and 20.5%,respectively(χ2=4.74,P=0.029).Multivariate Cox regression analysis indicated that CD36 expression,AFP expression,tumor differentiation grade and microvascular invasion were independent risk factors affecting the prognosis and survival of patients with HCC.Conclusion The expression level of CD36 is up-regulated significantly in HCC tissues,and is significantly related to AFP expression level,pathological differentiation grade,CNLC stage,microvascular invasion and prognosis,suggesting that CD36 plays an important role in the occurrence and prognosis of hepatocellular carcinoma.
