摘 要:目的 探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)、凝集素样氧化低密度脂蛋白清除受体1(LOX-1)的水平与颅内大动脉粥样硬化性狭窄预后的关系。方法 选取2020年1月至2022年12月商丘市第一人民医院收治的颅内大动脉粥样硬化性狭窄患者187例为研究对象,根据影像检查结果评估其狭窄程度,轻度狭窄61例、中度狭窄89例、重度狭窄37例。根据患者出院后1年内的随访结果评估其预后,预后良好162例,预后不良25例。于治疗前收集患者的静脉血,使用酶联免疫吸附法检测患者的血清Lp-PLA2、LOX-1水平。比较不同狭窄程度患者的血清Lp-PLA2、LOX-1水平以及预后良好组和预后不良组的血清Lp-PLA2、LOX-1水平。采用Spearman相关分析法分析颅内大动脉粥样硬化性狭窄患者的血清Lp-PLA2、LOX-1水平与狭窄程度的关系。采用受试者工作特征(ROC)曲线分析血清Lp-PLA2、LOX-1水平对颅内大动脉粥样硬化性狭窄患者预后不良的预测价值。结果 随着狭窄率的提高,患者治疗前血清Lp-PLA2及LOX-1的水平逐渐升高,且与颅内动脉狭窄程度之间存在正相关关系(r分别为0.725、0.698,P<0.05)。预后良好组的血清Lp-PLA2、LOX-1水平均低于预后不良组(P<0.05)。ROC曲线分析显示,血清Lp-PLA2、LOX-1、两者联合预测颅内大动脉粥样硬化性狭窄患者预后不良的AUC分别为0.777、0.760、0.904,联合预测的AUC显著高于两者单独预测(P<0.05)。结论 颅内大动脉粥样硬化性狭窄患者的血清Lp-PLA2、LOX-1水平均升高,其水平变化与狭窄程度以及预后关系密切。Objective To investigate the relationship between serum lipoprotein-associated phospholipase A2(Lp-PLA2), lectin-like oxidized low-density-lipoprotein receptor-1(LOX-1) levels and prognosis of intracranial atherosclerotic stenosis. Methods One hundred and eighty-seven patients with intracranial atherosclerotic stenosis admitted to Shangqiu First People's Hospital from January 2020 to December 2022 were selected, including 61 cases of mild stenosis, 89 cases of moderate stenosis, and 37 cases of severe stenosis. The prognosis of patients was assessed according to their follow-up within 1 year after discharge, including 162 cases of good prognosis and 25 cases of poor prognosis. Venous blood was collected before any treatment. The serum Lp-PLA2 and LOX-1 levels were detected by enzyme-linked immunosorbent assay, and were compared among patients of different degrees of stenosis and different status of prognosis. Spearman correlation analysis was conducted to explore the correlation between serum Lp-PLA2, LOX-1 levels and stenosis degree in patients with intracranial atherosclerotic stenosis. Then the receiver operating characteristic(ROC) curve was plotted to evaluate the value of serum Lp-PLA2 and LOX-1 in predicting the poor prognosis of patients with intracranial atherosclerotic stenosis. Results As the degree of stenosis aggravated, the levels of serum Lp-PLA2 and LOX-1 before treatment gradually increased, suggesting a positive correlation between the two indicators and the degree of stenosis(r=0.725 and 0.698, all P<0.05). The serum levels of Lp-PLA2 and LOX-1 in good prognosis group were lower than those in poor prognosis group(P<0.05). ROC curve analysis denoted that AUCs of the separate and combined test of serum Lp-PLA2 and LOX-1in predicting poor prognosis of intracranial atherosclerotic stenosis were 0.777, 0.760 and 0.904, showing a larger AUC under combined test than separate test(P<0.05).
