摘　　要：目的 探讨血清鸢尾素(Irisin)、白细胞介素-1β (IL-1β)及肿瘤坏死因子-α (TNF-α)对重症肌无力(MG)危象的预测价值。方法 选取2019年1月至2022年12月中国人民解放军联勤保障部队第九七〇医院神经内科收治入院的98例MG患者为MG组,根据是否发生MG危象分为危象组(12例)和非危象组(86例),另选取同期46例健康体检者为对照组。对比各组血清Irisin、IL-1 β、TNF-α水平,分析其与MG病情的相关性。观察血清Irisin、IL-1 β及TNF-α对MG危象的预测价值。结果 MG组血清Irisin水平显著低于对照组,血清IL-1 β及TNF-α水平高于对照组(P<0.05)。Spearman相关性分析显示,血清Irisin水平与MG病情呈负相关,而血清IL-1 β、TNF-α水平与病情严重程度呈正相关(P<0.05)。危象组血清Irisin水平低于非危象组,血清IL-1β、TNF-α水平则高于非危象组(P<0.05)。血清Irisin、IL-1 β及TNF-α联合预测MG危象的曲线下面积为0.895,敏感度为91.7%,特异度为76.7%。结论 血清Irisin、IL-1 β及TNF-α水平与MG病情严重程度有关,可作为早期预测MG危象的参考指标。Objective To investigate the predictive value of serum Irisin,interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)in myasthenia gravis(MC)crisis.Methods A total of 98 MG patients admitted to our hospital from January 2019 to December 2022 were selected as the MG group.According to the presence or absence of MG crisis,they were divided into crisis group(12 cases)and non-crisis group(86 cases).Another 46 healthy subjects were selected as the control group.The levels of serum Irisin,IL-1βand TNF-αin each group were compared,and their correlation with the severity of MG was analyzed.The predictive value of serum Irisin,IL-1βand TNF-αfor MG crisis was observed.Results The level of serum Irisin in MG group was lower than control group,and the levels of serum IL-1βand TNF-αwere higher than control group(P<0.05).Spearman correlation analysis showed that serum Irisin level was negatively correlated with the severity of MG,while serum IL-1βand TNF-αlevels were positively correlated with the severity of MG(P<0.05).The level of serum Irisin in crisis group was lower than non-crisis group,and the levels of serum IL-1βand TNF-αwere higher than non-crisis group(P<0.05).The area under the curve of serum Irisin,IL-1βand TNF-αcombined to predict MG crisis was 0.895,the sensitivity was 91.7%,and the specificity was 76.7%.Conclusion The levels of serum Irisin,IL-1βand TNF-αare related to the severity of MG,and can be used as reference indexes for early prediction of MG crisis.
