摘 要:目的了解临床护士的临床领导力、专业生活品质和护理服务质量的现状,探讨专业生活品质在临床领导力和护理服务质量间的中介作用。方法采用一般资料问卷、临床领导力量表、专业生活品质量表、护理服务质量评价量表对浙江省3所医联体基层医院的457名临床护士进行问卷调查。结果临床护士临床领导力总分为(64.09±9.58)分,专业生活品质的慈心满意得分为(40.11±7.56)分、职业倦怠得分为(29.39±8.77)分、二次创伤得分为23.88(16.00,30.00)分,护理服务质量总分为(86.30±22.47)分;护理服务质量与临床领导力呈正相关(r=0.836,P<0.01),与专业生活品质的慈心满意(r=0.846,P<0.01)呈正相关,与工作倦怠和二次创伤呈负相关(r=-0.463~-0.792,P<0.01),专业生活品质中慈心满意、职业倦怠和二次创伤在临床领导力和护理服务质量之间发挥部分中介作用,中介效应分别为0.568、0.123、0.373,分别占总效应的28.95%、6.27%、19.01%。结论临床护士专业生活品质仍有较大提升空间,护理管理者应采取多种干预措施培养临床护士临床领导力,提升其慈心满意,降低倦怠和二次创伤,进一步提高护理服务质量。Objective To understand the current status of clinical leadership,professional quality of life,and quality of care among clinical nurses,and to explore the mediating effect of professional quality of life between clinical leadership and care quality in clinical nurses.Methods A total of 457 clinical nurses from primary hospitals in three medical consortiums in Zhejiang Province were investigated by using General Data Questionnaire,Clinical Leadership Survey(CLS),Professional Quality of Life(ProQOL),and Quality of Nursing Care Scale(QNCS).Results The nurses scored(64.09±9.58)on CLS,(40.11±7.56)on compassionate satisfaction,(29.39±8.77)on occupational burnout,and 23.88(16.00,30.00)on secondary trauma of the ProQOL Scale,and(86.30±22.47)on the QNCS on average.The score of QNCS was positively correlated with CLS score(r=0.836,P<0.01).And it had a positive correlation with the score of compassionate satisfaction(r=0.846,P<0.01).But it had a negative correlation with the score of burnout and secondary trauma(r=-0.463,-0.792,P<0.01).Compassionate satisfaction,occupational burnout and secondary trauma partially mediated the relationship between clinical leadership and quality of nursing care,with the size of effect of 0.568,0.123 and 0.373,respectively,accounting for 28.95%,6.27%and 19.01%of the total effect,respectively.Conclusion There is still large room for improvement in the professional quality of life of clinical nurses.To further improve the quality of care,nursing managers should take multiple interventions to cultivate the clinical leadership,improve the compassionate satisfaction,reduce burnout and secondary trauma in clinical nurses.
