临床护士老年谵妄知信行现状及影响因素研究

摘　　要：目的了解临床护士老年谵妄相关知识、态度及行为现状,并分析其影响因素。方法采用自行设计的护士对老年谵妄知信行现状调查问卷对湖北省荆州市两所三级甲等综合医院的943名临床护士进行调查。结果护士老年谵妄知信行总分为(178.34±33.38)分。知识得分为(75.43±22.70)分,合格率为31.18%;态度得分为(37.23±5.48)分,合格率为90.14%;行为得分为(65.69±11.83)分,合格率为75.19%。多元线性回归分析显示,轮转科室和专科护士是护士老年谵妄知识的主要影响因素(P<0.05),未检测到护士老年谵妄态度和行为的主要影响因素。结论临床护士老年谵妄知信行总体水平不高,知识水平偏低,态度较积极,行为水平有待提高。护理管理者应对护士进行老年谵妄知识培训,帮助其树立正确、积极的态度,改善护士老年谵妄相关行为,从而有效地预防和护理老年谵妄。Objective To understand the level and associated factors of clinical nurses'knowledge,attitude and practice(KAP)about delirium in the elderly.Methods A self-designed questionnaire on the knowledge,attitude and practice status of eldly delirium among nurses was used to investigate 943 clinical nurses from two grade A tertiary general hospitals in Jingzhou,Hubei Province.Results The mean total score of nurses'KAP was(178.34±33.38).The mean score of the knowledge part was(75.43±22.70),and 31.18%of the nurses reached the eligibility criterion.The mean score of the attitude part was(37.23±5.48),and 90.14%of the nurses reached the eligibility criterion.The mean score of the practice part was(65.69±11.83),and 75.19%of the nurses reached the eligibility criterion.Multiple linear regression analysis showed that experiences of rotation between various departments and working as a specialist nurse were main factors affecting the nurses'knowledge of delirium in the elderly(P<0.05).However,no main factors affecting the nurses'attitude and practice towards delirium in the elderly were detected.Conclusion The overall level of clinical nurses'KAP about delirium in the elderly is not high,in particular,the level of knowledge is low,but the attitude is positive,and the level of practice needs to be improved.In view of this,nursing managers should provide relevant knowledge trainings for nurses,help them develop correct and positive attitude,and improve nurses'practice related to delirium in the elderly,so as to effectively conduct interventions for the prevention and nursing of delirium in the elderly.
