摘　　要：目的:探究采用光动力治疗(Photodynamic Therapy,PDT)在口腔黏膜癌前病变口腔白斑病治疗中的应用,并总结相关临床经验。方法:利用特定波长激光照射经20%的5-氨基酮戊酸外用散(5-Aminolevulinic Acid,ALA)作为光敏剂孵育2 h的病损部位20 min。在接受病理检查确诊后,对24例口腔白斑病患者实施光动力疗法,观察评估其有效性和安全性。结果:所治病例经PDT治疗后的总有效率87.5%。由临床观察的数据统计结果采用Pearson和Spearman相关分析可见,在治愈及治疗显效的21例患者中,病损面积越大,所需治疗时间越长(r=0.84,P<0.001);PDT治疗病损区域单位面积所需次数与病理分型未观察到显著相关(r=-0.146,p=0.527)。结论:光动力治疗口腔黏膜癌前病变白斑病,疗效高,效果明确、组织损伤小、微创,且可通过调节免疫有效预防复发及癌变。应用此治疗可为精准治疗口腔白斑病提供新的方法,具有较好的应用价值。Objective:To explore the application of Photodynamic Therapy(PDT)in the treatment of oral leukoplakia,a precancerous lesion of oral mucosa,in order to summarize relevant clinical experience.Methods:The efficacy and safety of PDT were observed and evaluated in 24 patients with oral leukoplakia who were diagnosed by biopsy.Lesions that had been incubated for 2 hours with 20%5-Aminolevulinic Acid(ALA)as a photosensitizer were irradiated with a specific wavelength laser for 20 minutes.Results:After PDT,the total efficiency rate of all patients was 87.5%.The statistical results of clinical observation can be seen by Pearson and Spearman correlation analysis.In 21 patients who were cured or effective significantly in treatment,the greater the lesion area,the longer the treatment time(r=0.84,P<0.001).There was no significant correlation between the number of times per unit area of PDT and pathological classifiction(r=-0.146,P=0.527).Conclusion:PDT of oral mucosal precancerous lesions has high efficacy,clear effect,little tissue damage,minimal invasion,and may prevent recurrence effectively and canceration by regulating immunity.The application of this treatment can provide a new method for accurate treatment of oral leukoplakia,and it has a good application value.
