血清GP73、CEA及CA7-24在结肠癌诊断中的价值探讨

摘　　要：目的分析血清高尔基体糖蛋白73(GP73)、癌胚抗原(CEA)、糖类抗原72-4(CA72-4)在结肠癌诊断中的应用价值。方法选取2021年2月至2023年3月本院收治的49例结肠癌手术患者纳入结肠癌组,同期在本院体检的50例健康者作为对照组,比较两组血清GP73、CEA、CA72-4水平和阳性率,分析结肠癌组血清中GP73、CEA、CA72-4单独及联合检测的敏感度、特异度,分析诊断价值。结果结肠癌组血清GP73、CEA、CA72-4水平均明显高于健康对照组,差异均有统计学意义(均P<0.01)。结肠癌组单一血清检测的阳性率和3项指标联合检测的阳性率均高于对照组,差异均有统计学意义(均P<0.01)。ROC曲线分析结果显示,血清GP73、CEA、CA72-4及联合检测均对结肠癌有较高的诊断价值,AUC值分别为0.830、0.903、0.843、0.959,单一血清检测敏感度最好的是CEA和CA72-4,特异度最高的是GP73,3项指标联合检测的敏感度和特异度分别为99.32%、84.18%。结论GP73、CEA及CA72-4检测对诊断结肠癌具有一定价值,3项指标联合检测能够弥补单项检测的不足,价值较高。Objective To analyze the application value of serum Golgi protein 73(GP73),carcinoembryonic antigen(CEA),and carbohydrate antigen 72-4(CA72-4)in the diagnosis of colon cancer.Methods Forty-nine patients with colon cancer were selected as the colon cancer group,and 50 healthy individuals undergoing physical examinations were se-lected as the control group.The differences in serum levels and positive rates of GP73,CEA,and CA72-4 between the two groups were compared.Additionally,the differences in preoperative and postoperative serum levels of GP73,CEA,and CA72-4 in the colon cancer group were compared.The sensitivity and specificity of GP73,CEA,and CA72-4 alone and in combination were analyzed,and ROC curve analysis was performed.Results The serum levels of GP73,CEA,and CA72-4 in the colon cancer group were significantly higher than those in the control group(P<0.01).There were signifi-cant differences in the positive rates of single serum markers and the combination of the three markers between the two groups(P<0.01).Preoperative serum levels of GP73,CEA,and CA72-4 were significantly higher than postoperative lev-els(P<0.01).ROC curve analysis showed that serum GP73,CEA,and CA72-4 had good diagnostic value for colon cancer(AUC=0.830,0.903,0.843,all P<0.01).The AUC for combined detection of GP73+CEA+CA72-4 was 0.959.The best sensitivity for single marker detection was CEA and CA72-4,the highest specificity was GP73.The sensitivity and specificity of the combined detection of the three markers were 99.32%and 84.18%.Conclusion GP73,CEA,and CA72-4 have a certain diagnostic value for colon cancer,and the combined detection of the three markers can compensate for the deficiencies of single-marker detection,which is of great significance in improving the diagnostic efficiency of colon cancer.
