95部颁标准中的二十五味大汤方剂治疗胃溃疡出血的原理和方解研究

摘　　要：目的 应用95部颁标准中的二十五味大汤方剂治疗胃溃疡出血,探讨其临床治疗原理,并进行方解分析研究。方法 于2021年8月—2022年8月选取于本院诊断为胃溃疡出血的患者72例作为研究对象,按照随机数字表法分为对照组(36例)与观察组(36例),其中对照组采用奥美拉唑,观察组采用95部颁标准中的二十五味大汤方剂进行治疗。观察两组患者治疗期间的血清指标与胃肠激素水平变化,并对二十五味大汤方剂所用藏药的性味和性能进行统计分析,对其进行方解研究。结果 与治疗前比较,治疗后两组患者的SOD、EGFR以及血清内炎症因子表达水平均显著升高(P<0.05),其中以观察组患者在治疗后血清内各指标表达水平升高情况更为显著(P<0.05)。治疗后两组患者的胃肠激素水平均较治疗前降低(P<0.05),其中观察组降低水平更为显著(P<0.01)。结论 95部颁标准中的二十五味大汤方剂可通过改善患者胃部炎症反应与溃疡出血等情况达到愈溃疡止血作用,且各藏药性味以苦、寒、温等为主,具有收敛止血、和胃止痛等作用,具有较高应用价值。Objective To apply the Twenty-Five Flavor Decoction in the 1995 edition of drug standards issued by the Ministry of Health in the treatment of gastric ulcer bleeding,explore its clinical treatment principle,and analyze the formulation.Methods From August 2021 to August 2022,72 patients diagnosed with gastric ulcer bleeding in the Tibetan Hospital of Angren County were randomly selected as the research objects.They were randomly divided into a control group(n = 36) and an observation group(n = 36) according to different treatment methods.The patients in the control group received omeprazole,and those in the observation group were treated with Twenty-Five Flavor Decoction in the 1995 edition of drug standards issued by the Ministry of Health.The changes in serum indexes and gastrointestinal hormone levels of the two groups were observed during the treatment,and the flavors and properties of the Tibetan drugs used in the Twenty-Five Flavor Decoction were statistically analyzed,with the formulation studied.Results After treatment,the levels of SOD,EGFR,and serum inflammatory factors in the two groups significantly increased(P < 0.05),and the levels of serum indexes in the observation group were significantly higher than those before treatment(P < 0.05).In addition,the gastrointestinal hormone levels of the two groups after treatment were significantly lower than those before treatment(P < 0.05),and the reduction level of the observation group was more significant(P < 0.01).Conclusion The Twenty-Five Flavor Decoction in the 1995 edition of drug standards issued by the Ministry of Health can heal ulcers and stop bleeding by improving gastric inflammatory reactions and ulcer bleeding in patients.The main flavors and properties of the Tibetan drugs are bitter,cold,and warm,and they have the effects of astringing to stop bleeding,relieving pain,and harmonizing the stomach,which has high application value.
