MMR蛋白、p53、HER-2与Ki-67在子宫内膜癌组织中表达及与临床病理特征的关系

摘　　要：目的探讨错配修复(MMR)蛋白、抑癌基因p53(p53)、表皮生长因子受体2(HER-2)与肿瘤增殖抗原(Ki-67)在子宫内膜癌组织中表达及与临床病理特征的关系。方法选取濮阳市人民医院2019年1月至2021年1月收治的100例(均完成随访)子宫内膜癌患者为研究对象,采用免疫组化法检测子宫内膜癌组织和癌旁组织中MMR蛋白(MLH1、PMS2、MSH2、MSH6)、p53、HER-2、Ki-67表达情况,比较不同病理学参数子宫内膜癌组织中MMR、p53、HER-2、Ki-67的表达情况,分析子宫内膜癌组织中MMR、p53、HER-2、Ki-67表达与临床病理学参数的关系,分析MMR、p53、HER-2、Ki-67表达与子宫内膜癌患者预后关系。结果子宫内膜癌组织中MMR表达缺失率、p53、HER-2、Ki-67阳性表达率高于癌旁组织(P<0.05);不同病理类型患者子宫内膜癌组织MMR表达缺失率、p53、HER-2、Ki-67阳性表达率比较:Ⅰ~Ⅱ期低于Ⅲ~Ⅳ期,高分化<中分化<低分化,无淋巴结转移低于有淋巴结转移(P<0.05);子宫内膜癌组织MMR表达缺失率、p53、HER-2、Ki-67阳性表达率与临床分期、淋巴结转移均呈正相关,与组织分化程度呈负相关(P<0.05);MMR表达缺失、p53、HER-2、Ki-67阳性子宫内膜癌患者2年生存率分别低于MMR表达完整、p53、HER-2、Ki-67阴性子宫内膜癌患者(P<0.05)。结论MMR蛋白、p53、HER-2、Ki-67表达情况与子宫内膜癌发生发展显著相关,且在预测预后方面具有积极意义。Objective To investigate the expression of mismatch repair(MMR)protein,tumor suppressor gene p53(p53),epidermal growth factor receptor 2(HER-2)and tumor proliferation antigen(Ki-67)in endometrial carcinoma tissues and their relationship with clinicopathologic features.Methods Totally 100 patients with endometrial cancer admitted to Puyang People's Hospital from January 2019 to January 2021(all completed follow-up)were selected as research objects.Immunohistochemical method was used to detect the expression of MMR protein(MLH1,PMS2,MSH2,MSH6),p53,HER-2 and Ki-67 in endometrial cancer tissues and adjacent tissues.The expressions of MMR,p53,HER-2 and Ki-67 in endometrial cancer tissues with different pathological parameters were compared,and the relationship between the expressions of MMR,p53,HER-2 and Ki-67 in endometrial cancer tissues and clinicopathological parameters was analyzed.To analyze the relationship between the expression of MMR,p53,HER-2,Ki-67 and prognosis of patients with endometrial cancer.Results The deletion rate of MMR expression,the positive expression rates of p53,HER-2 and Ki-67 in endometrial carcinoma tissues were higher than those in paracancer tissues(P<0.05).The expression deletion rate of MMR,positive expression rates of p53,HER-2 and Ki-67 in endometrial cancer tissues of different pathological types were lower than that of stageⅢtoⅣ,highly differentiated<medium differentiated<low differentiated,no lymph node metastasis was lower than that of lymph node metastasis(P<0.05).The deletion rate of MMR expression,positive expression rate of p53,HER-2 and Ki-67 were positively correlated with clinical stage and lymph node metastasis,and negatively correlated with the degree of tissue differentiation(P<0.05).The 2-year survival rate of patients with MMR deletion,p53,HER-2 and Ki-67 positive endometrial cancer was lower than that of patients with intact MMR expression,p53,HER-2 and Ki-67 negative endometrial cancer(P<0.05).Conclusion The expressions of MMR protein,p53,HER-2 and Ki-67 are sig
