儿童肾母细胞瘤治疗与预后分析

摘　　要：目的通过对肾母细胞瘤的治疗及预后进行分析,以期为患儿在治疗过程中获益更多。方法回顾性分析2012年6月至2021年10月南方医科大学附属佛山妇幼保健院及汕头大学医学院第二附属医院收治的12例肾母细胞瘤患儿的临床诊治资料。利用Kaplan-Meier曲线模型进行生存分析。对比患儿化疗前后的体重发育曲线,并进行线性回归分析,对比两者的曲线斜率。根据化疗过程中是否进行营养支持分成有营养支持组及无营养支持组,分别将两组患儿体重发育曲线与正常生长发育曲线进行分析,对比三者之间的斜率。结果患儿儿童肿瘤协作组分期、肿瘤直径、病理分型等单因素比较差异均无统计学意义(P>0.05)。在无肠内外营养支持的情况下,患儿化疗前后体重曲线斜率明显低于正常儿童生长发育曲线斜率,两者比较差异有统计学意义(P<0.05);在有肠内外营养支持的情况下,两者曲线斜率比较差异无统计学意义(P>0.05)。结论本研究对肾母细胞瘤患儿的生存概率进行单因素分析,发现肿瘤大小、肿瘤分期及病理分型暂未发现对患儿的存活有明显影响。在化疗过程中,应用肠内外营养支持有利于维持患儿正常发育,可考虑在此过程中适时应用。Objective To summarize the clinical experience of Wilms tumor in order to benefit children more in the process of treatment.Methods The clinical data of 12 cases with Wilms tumor from June 2012 to October 2021 were reviewed.Kaplan-Meier curve was used to analyze the relationship between univariate survival and prognosis.The growth and development curves of children before and after chemotherapy were compared,and the slopes of the two were compared by linear regression analysis.Results There was no significant difference in single factor such as COG stage,tumor diameter or pathological classification(P>0.05).Without nutritional support,the slope of body weight curve before and after chemotherapy was significantly lower than that of normal children's growth and development curve(P<0.05).With nutritional support,there was no significant difference between the slopes of the two curves(P>0.05).Conclusions In this study,Single factor analysis was used to analyze the survival of children with nephroblastoma,suggesting that tumor size,tumor stage and pathological classification have not significantly affected the survival of children.In the process of chemotherapy,the application of enteral and parenteral nutritional support can be conducive to maintaining the normal development of children.It can be considered to be applied timely in this process.
