五苓散与栝楼瞿麦丸的证治比较研究

摘　　要：《金匮要略》是我国现存最早的一部论述杂病诊治的专书,它的问世不仅奠定了中医学的基础,也为后世中医理论的发展以及方书涌现开创了先河。在《金匮要略》原文中,五苓散证与栝楼瞿麦丸证的主诉均出现了口渴与小便不利的症状,但在临床上的具体应用情况却有很大的区别。五苓散治疗的是太阳病表邪不解,外邪入里,波及了太阳之腑膀胱,见到膀胱气化不行的小便不利之证,是一个表里双解之方,需要通阳化气,以恢复机体的气化功能;栝楼瞿麦丸证则是在膀胱蓄水证的基础上出现了口苦口渴的症状,集中表现为阳虚水停、津液匮乏、火邪上炎的问题,需要温补肾阳、生津止渴,以振奋阳气,畅达津液。在治疗时,对基础理论知识的把握、对四诊得到信息的整合,以及对各味药物的理解与认识,都会影响对疾病的判断,临证时不可为疾病的表象所惑。Jingui Yaolue(《金匮要略》)is the earliest book on the diagnosis and treatment of miscellaneous diseases in China,its appearance not only laid the foundation of TCM,but also created a precedent for the development of TCM theory and the emergence of prescription books.In the original of Jingui Yaolue,the chief complaints of Wuling San(五苓散)syndrome and Gualou Qumai Wan(栝楼瞿麦丸)syndrome both showed symptoms of thirst and unfavorable urination,however,there are great differences in the specific clinical application.Wuling San is used to treat symptoms which the evil spirit outside enters and affects bladder,causing poor urination,what it needs is restoring the body’s function of promoting the circulation of Qi(气).On the basis of this,the latter shows bitter tongue more.Due to the deficiency of kidney and body fluid,it needs to be treated by tonifying the kidney and promoting fluid production.In terms of treatment,the understanding of the basic theoretical knowledge,the integration of the information obtained from the four diagnoses,and the understanding of TCM materials will affect the judgment of the disease,and the doctor should not be confused by the characterization of the disease.
