摘 要:目的:探究磁共振成像技术(MRI)在原发性三叉神经痛(ITN)患者神经血管压迫评估中的应用效果。方法:选取2019年1月—2023年5月上海交通大学医学院附属仁济医院浦南分院收治的ITN患者80例为研究对象,使用MRI对患者进行检查,以手术检查结果为“金标准”,分析MRI对于神经血管压迫的诊断结果。结果:有症状侧存在压迫关系率高于无症状侧,未见神经血管接触率低于无症状侧,差异有统计学意义(P<0.001);MRI检查与手术检查三叉神经出脑干区与神经血管压迫点的最短距离比较,差异无统计学意义(P>0.05);以手术检查结果为“金标准”,MRI的诊断准确度为92.50%,灵敏度为93.33%,特异度为80.00%。结论:神经血管压迫部位和程度影响ITN患者是否出现症状,应用MRI技术评估和诊断ITN患者的神经血管压迫准确性较好,可用于ITN患者的术前神经血管压迫评估。Objective:To explore the effect of magnetic resonance imaging(MRI)in the evaluation of neurovascular compression in patients with primary trigeminal neuralgia(ITN).Methods:A total of eighty patients with ITN admitted to Punan Branch of Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2019 to May 2023 were selected as the study objects,and MRI was used to examine the patients.With the results of surgical examination as the"gold standard",the diagnostic results of neurovascular compression by MRI were analyzed.Results:The compression relationship rate in symptomatic side was higher than that in asymptomatic side,and the neurovascular contact rate in asymptomatic side was lower than that in asymptomatic side,the difference was statistically significant(P<0.001);there was no significant difference in the shortest distance between the trigeminal nerve outlet area and neurovascular compression point between MRI and surgical examination(P>0.05);with surgical examination results as the"gold standard",the diagnostic accuracy of MRI was 92.50%,the sensitivity was 93.33%,and the specificity was 80.00%.Conclusion:The location and degree of neurovascular compression affect whether ITN patients have symptoms.The application of MRI technology to evaluate and diagnose neurovascular compression in ITN patients has good accuracy,and can be used for preoperative neurovascular compression evaluation in ITN patients.
