武汉市青少年自杀相关行为危险因素调查研究

摘　　要：目的本研究旨在探讨武汉市青少年自杀相关行为的流行现状及其危险因素,为青少年自杀预防提供参考依据。方法以武汉市青少年为研究对象,使用结构化问卷调查其健康相关行为,采用二元Logistic回归分析方法探讨青少年自杀相关行为的危险因素。结果共调查武汉市青少年840人,青少年自杀意念、自杀计划和自杀未遂发生率分别为13.7%、6.2%和5.2%。在自杀意念方面,高中学段(OR=5.07,95%CI:2.25~11.44)、受到父母忽视(OR=2.20,95%CI:1.30~3.73)、经历过父母间家暴(OR=3.64,95%CI:1.97~6.70)、经历过成年人性骚扰(OR=4.75,95%CI:1.71~13.18)以及经常处于情绪低落状态(OR=3.43,95%CI:1.65~7.15)的青少年产生自杀意念的风险更高。睡眠情况是青少年自杀计划的独立影响因素,失眠的青少年制定自杀计划的风险更高(OR=2.79,95%CI:1.17~6.62)。学段是青少年自杀未遂的独立影响因素,高中学段的青少年进行自杀未遂的风险更高(OR=3.97,95%CI:1.35~11.61)。结论武汉市青少年所在学段、家庭环境、成年人性骚扰以及青少年情绪状态是其自杀意念的影响因素;失眠和学段分别对青少年自杀计划和自杀未遂存在独立影响。Objective To investigate the prevalence of suicide-related behaviors and its influencing factors among adolescents in Wuhan City.Methods A total of 840 adolescents in Wuhan City were selected as research objects.A structured questionnaire was used to investigate their health-related behaviors,and binary logistic regression analysis was used to explore the risk factors of adolescent suicide-related behaviors.Results The incidence of suicide ideation,suicide plan and attempted suicide were 13.7%,6.2%and 5.2%,respectively.In terms of suicidal ideation,high school(OR=5.07,95%CI:2.25-11.44),parental neglect(OR=2.20,95%CI:1.30-3.73),parental violence(OR=3.64,95%CI:1.97-6.70),adult sexual harassment(OR=4.75,95%CI:1.71-13.18)and frequently depressed adolescents(OR=3.43,95%CI:1.65-7.15)had higher risk of suicidal ideation.In addition,sleep status was an independent factor in adolescent suicide planning,and adolescents with insomnia had higher risk of making suicide plans(OR=2.79,95%CI:1.17-6.62).Learning stages were independent determinants of attempted suicide.The adolescents in senior high school had higher risk of attempted suicide(OR=3.97,95%CI:1.35-11.61).Conclusion he suicidal behaviors of adolescents in Wuhan should not be ignored.The learning stages,family environment,sexual harassment of adults and emotional state of adolescents were independent factors associated to their suicidal ideation.Insomnia and learning stages were independent factors of suicide planning and attempted suicide,respectively.
