“从阳引阴”法针刺治疗脑卒中后足内翻临床疗效

摘　　要：目的:探讨“从阳引阴”法针刺治疗脑卒中后足内翻的效果。方法:选择针灸科2019年1月-2021年5月收治的脑卒中后足内翻患者96例,按照随机数字表法分为治疗组和对照组,每组48例。两组均接受脑血管病常规治疗,对照组根据《针灸学》中脑卒中针灸处方予以针刺治疗,治疗组予以“从阳引阴”法针刺治疗。疗程均为4周。比较两组治疗前后足内翻角度、简式Fugl-Meyer运动功能评分法(Fugl-Meyer Assessment Scale,FMA)评分,以及步态分析指标步速、步频和步幅,并比较两组胫骨前肌厚度、胫骨后肌厚度和治疗总有效率。结果:治疗后,治疗组足内翻角度小于对照组,下肢FMA评分高于对照组,步速、步频和步幅均高于对照组,胫骨前肌厚度和胫骨后肌厚度高于对照组,总有效率高于对照组,差异均有统计学意义(P <0.05)。结论:“从阳引阴”法针刺可促进胫骨前肌、胫骨后肌厚度的恢复,减小足内翻角度,改善下肢运动功能,对于脑卒中后足内翻效果显著。Objective:To investigate the effect of acupuncture at Yang(阳)points for Yin(阴)diseases on foot varus after stroke.Methods:A total of 96 patients with foot varus after stroke from January 2019 to May 2021 were divided into the treatment group and the control group according to the random number table method,with 48 cases in each group.Both groups received routine treatment for cerebrovascular diseases.On the basis of the above treatment,the control group received acupuncture treatment according toO the stroke acupuncture recipe in acupuncture and moxibustion,while the treatment group were treated by acupuncture at Yang points for Yin diseases.The course of treatment was for 4 weeks.Foot varus angle,FMA score,and gait analysis indexes of step speed,step frequency and step length were compared between the two groups before and after treatment.The tibialis anterior muscle thickness,tibialis posterior muscle thickness,and the total efficiency of the treatment were compared between the two groups.Results:After treatment,the foot varus angle in the treatment group was smaller than that in the control group,the FMA scores of lower limbs in the treatment group was higher than that in the control group,the step speed,step frequency,and step length in the treatment group were higher than those in the control group,the tibialis anterior muscle thickness and tibialis posterior muscle thickness were higher than the control group,the total efficiency in the treatment group was higher than the control group,and the differences were all statistically significant(P<0.05).Conclusion:Acupuncture at Yang point for Yin disease can promote the recovery of the thickness of tibialis anterior muscle and tibialis posterior muscle,reduce the angle of foot varus,improve the motor function of lower limbs,and it has a significant effect on foot varus after stroke.
