摘 要:目的探讨宫颈环形电切术(LEEP)在高级别子宫颈上皮内瘤变(CIN)患者的应用价值。方法回顾性选取2016年9月至2018年6月于长沙生殖医学医院行宫颈锥切术的65例高级CIN病变患者作为研究对象,按照手术方式不同分为LEEP组(n=49)与冷刀锥切术(CKC)组(n=16)。LEEP组行LEEP术,CKC组行CKC术,比较两组围手术期指标、术后并发症发生情况、临床疗效。结果LEEP组手术时间、创面愈合时间均短于CKC组,术中出血量少于CKD组,差异有统计学意义(P<0.05);两组切缘阳性率、复发率、治愈率比较差异均无统计学意义。两组术后相关并发症发生率比较差异无统计学意义。结论LEEP治疗高级别宫颈上皮内瘤变患者疗效确切,手术时间、创面愈合时间均较短,术中出血量较少,值得临床推广应用。Objective To evaluate the clinical value of loop electrosurgical excision procedure(LEEP)in patients with high-grade cervical traepithelial neoplasia(CIN).Methods 65 patients with advanced CIN lesions who underwent cervical conectomy in Changsha Reproductive cine Hospital from September 2016 to June 2018 were retrospectively selected as the study subjects,and they were divided into the LEEP group(n=49)and the cold-knife conization(CKC)group(n=16)according to different surgical methods.The LEEP group received LEEP and CKC group re-ceived CKC,the perioperative indexes,postoperative complications and clinical efficacy were compared between the two groups.Results The oper-ative time and wound healing time in the LEEP group were shorter than those in the CKC group,and the intraoperative blood loss was less than the CKD group,the differences were statistically significant(P<0.05);there were no significant differences in the positive rate,recurrence rate and cure rate between the two groups.There was no significant difference in the incidence of postoperative complications between the two groups.Conclu-sion LEEP is effective in the treatment of high-grade cervical intraepithelial neoplasia,with shorter operative time and wound healing time,and less intraoperative blood loss.It is worthy of clinical promotion and application.
