摘　　要：目的 探讨含膳食纤维的肠内营养联合益生菌对老年重症脓毒症患者肠黏膜屏障的影响观察。方法 选取河北省优抚医院重症医学科2020年10月~2022年12月收治的60例老年重症脓毒症患者,按照随机数表法分为观察组(n=30)、对照组(n=30),两组均给予清创引流、液体复苏、抗感染、机械通气等基础治疗,对照组给予含膳食纤维的肠内营养治疗,观察组联合益生菌治疗,对比分析两种治疗方式对老年重症脓毒症患者肠黏膜屏障的影响。结果 治疗后,观察组D-乳酸、二胺氧化酶(DAO)、内毒素、血糖平均值(GLUave)、最大血糖变异幅度(LAGE)水平均明显低于对照组,差异有统计学意义(P<0.05);观察组总蛋白(TP)、前白蛋白(PA)、白蛋白(ALB)水平均高于对照组,差异有统计学意义((P<0.05)。结论含膳食纤维的肠内营养联合益生菌在老年重症脓毒症患者中应用较佳,可显著改善患者营养状态,促进肠道黏膜屏障功能恢复,降低血糖波动,值得临床应用。Objective To investigate the effect of enteral nutrition containing dietary fiber combined with probiotics on intestinal mucosal barrier in elderly patients with severe sepsis.Methods 60 elderly patients with severe sepsis admitted by the department of critical medicine of Hebei Youfu Hospital from October 2020 to December 2022 were randomly divided into observation group(n=30)and control group(n=30).Both groups were given basic treatments and enteral nutrition containing dietary fiber,and the observation group was treated with probiotics on this basis.Intestinal mucosal barrier in the two groups was comparatively analyzed.Results After treatment,D-lactic acid,diamine oxidase(DAO),mean blood glucose(GLUave)and maximum blood glucose variability(LAGE)in the observation group were significantly lower than those of the control group(P<0.05).After treatment,the levels of total protein(TP),prealbumin(PA)and albumin(ALB)in the observation group were higher than those in the control group(P<0.05).Conclusion Applying enteral nutrition containing dietary fiber combined with probiotics in elderly patients with severe sepsis can significantly improve the patients′nutritional status,promote the recovery of intestinal mucosal barrier function,and reduce blood glucose fluctuation.
