参附注射液对离体空跳猪心血液保护的研究

摘　　要：目的 探讨参附注射液对离体空跳猪心的血液保护作用。方法 将12头健康广西巴马小型猪随机分为参附组和对照组,每组6头。2组建立离体空跳心脏模型,参附组在预充液中加入10 mL参附注射液,体外循环建立后,经膜式氧合器,微量泵泵注参附注射液5 mL/h;对照组体外循环建立后,经膜式氧合器,微量泵泵注等量的0.9%氯化钠注射液。2组均持续泵入8 h。记录2组猪心脏复跳时间及体外循环8 h时平均动脉压、心率、灌注流量、灌注转速、血温,检测2组体外循环建立初及体外循环2 h、4 h、6 h、8 h的血常规(白细胞、红细胞、血小板计数)、凝血功能[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-二聚体(D-dimer)]和炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)]水平。结果 2组心脏复跳时间及体外循环8 h时平均动脉压、心率、灌注流量、灌注转速、血温比较差异均无统计学意义(P均>0.05)。与对照组比较,参附组体外循环后各时间点白细胞计数和D-dimer、IL-6、TNF-α水平均明显降低(P均<0.05),红细胞计数、血小板计数及FIB、IL-10水平均明显升高(P均<0.05),PT明显缩短(P<0.05),2组APTT、TT比较差异均无统计学意义(P均>0.05)。结论 参附注射液能够减少离体空跳猪心体外循环灌注过程中血细胞的破坏,改善凝血功能,抑制炎症反应的发生,有一定的血液保护作用。Objective It is to investigate the effect of Shenfu injection in the blood protection of isolated empty-beating pig hearts.Methods Twelve healthy Guangxi Bama miniature pigs were randomly divided into Shenfu group and control group,with 6 pigs in each group.The isolated empty-beating heart models were established in both groups,10 mL of Shenfu injection was added to the pre-filled fluid in the Shenfu group,and after establishment of the extracorporeal circulation,the Shenfu injection was injected with 5 mL/h by micropump in membrane oxygenator.The control group was injected with an equal amount of 0.9%sodium chloride injection after establishment of the extracorporeal circulation.Both groups were continuously pumped for 8 h.The time of cardiac resuscitation,the mean arterial pressure,heart rate,perfusion flow rate,perfusion rotation speed,and blood temperature of the two groups after 8 h of extracorporeal circulation were recorded,and the levels of blood routine indexes(leukocyte,erythrocyte,and platelet counts),coagulation function[prothrombin time(PT),activated partial thromboplastin time(APTT),prothrombin time(TT),fibrinogen(FIB),D-dimer]and inflammatory factors[interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α),interleukin-10(IL-10)]of the two groups were detected after establishment of the extracorporeal circulation 2 h,4 h,6 h,8 h of extracorporeal circulation.Results There was no significant difference in cardiac restart time,mean arterial pressure,heart rate,perfusion flow rate,perfusion rotation speed and blood temperature after 8 h of extracorporeal circulation between the two groups(all P>0.05).Compared with the control group,the leukocyte count and the levels of D-dimer,IL-6,and TNF-αwere significantly lower,the erythrocyte count,platelet count,and the levels of FIB and IL-10 were significantly higher,and the PT was significantly shorter at all time points after the extracorporeal circulation in the Shenfu group(all P<0.05),and the differences in APTT and TT were not statistically significant be
