早期肾癌采用不同入路腹腔镜下肾部分切除术的疗效观察

摘　　要：目的采用差异化入路腹腔镜肾部分切除术对早期肾癌患者进行治疗,分析临床疗效。方法选取2021年4月—2022年4月南阳市中心医院收治的66例早期肾癌患者,依据腹腔镜入路的差异性分为两个小组各33例,分别是经腹组和后腹腔组,对比两组患者的治疗效果。结果两组患者临床疗效(经腹组84.85%,后腹腔镜组90.91%)差异无统计学意义(P>0.05)。两组患者热缺血时间、术中出血量、标本重量以及肾血管控制时间方面的差异不存在统计学意义(P>0.05),两组患者在手术时间和恢复进食时间方面后腹腔镜组优于经腹组(P<0.05)。经过腹腔镜肾部分切除术治疗之后,后腹腔镜组的疼痛评分明显低于经腹组(P<0.05)。不同入路方式治疗的患者在并发症方面的发生率(经腹组9.09%,后腹腔镜组12.12%)差异无统计学意义(P>0.05)。结论不同入路的腹腔镜肾部分切除术治疗方式均具有自身优势和缺点,但是后腹腔入路可降低机体应激反应和疼痛程序,促进患者快速康复。Objective Differential approach laparoscopic partial nephrectomy was used to treat patients with early renal cancer,and the clinical effect was analyzed.Methods A total of 66 patients with early stage renal cancer admitted to our hospital from April 2021 to April 2022 were selected and divided into two groups with 33 cases in each group according to the difference of laparoscopic approach,namely the transabdominal group and the retroabdominal group.The treatment effects of the two groups were compared.Results There was no significant difference in clinical efficacy between the two groups(84.85%)transabdominal group and 90.91%in retrolaparoscopic group (P>0.05).There were no statistically significant differences between the two groups in terms of hot ischemia time,intraoperative blood loss,sample weight and renal vascular control time(P>0.05),and the retrolaparoscopy group was superior to the transabdominal group in terms of operation time and feeding time(P<0.05).After laparoscopic partial nephrectomy(LPN)treatment,the pain score of the retroperitoneal group was significantly lower than that of the transabdominal group(P<0.05).There was no significant difference in the incidence of complications among patients treated with different approaches(9.09%in the transabdominal group and 12.12%in the retrolaparoscopic group)(P>0.05).Conclusion LPN treatment with different approaches has its own advantages and disadvantages,but the retroperitoneal approach can reduce the body stress response and pain process,and promote rapid recovery of patients.
