摘  要:目的探讨可溶性细胞间黏附分子-1(sICAM-1)、C反应蛋白(CRP)、基质金属蛋白酶-12(MMP-12)表达情况与结肠癌合并糖尿病患者围术期血糖水平的关系及术后感染的预测价值。方法分析徐州医科大学附属宿迁医院2020年1月至2021年6月纳入的98例结肠癌合并糖尿病患者,以是否发生术后感染为依据,分为感染组(n=40)及未感染组(n=58)。观察两组的一般资料,比较术前、术后7 d两组的sICAM-1、CRP、MMP-12、血糖水平,并运用Logistic回归分析术后发生感染的影响因素,通过受试者工作特征曲线(ROC)评估血糖控制情况与sICAM-1、CRP、MMP-12水平对术后发生感染的预测价值。结果两组间围手术期血糖控制情况与sICAM-1、CRP、MMP-12水平比较,差异有统计学意义(P<0.05);术后7 d感染组的sICAM-1、CRP、MMP-12、血糖水平高于未感染组,差异有统计学意义(t=36.466、8.491、8.103、2.045,P均<0.05);通过多因素Logistic回归分析结果显示围手术期血糖控制不佳与sICAM-1≥211.0μg/L、CRP<0.8 mg/L或者>8.0 mg/L、MMP-12<80.0 mg/L或者>140.0 mg/L水平是影响术后发生感染的独立危险因素,差异有统计学意义(P<0.05);ROC曲线结果显示,血糖控制情况与sICAM-1、CRP、MMP-12水平四者联合检测诊断价值优于各项单独检测。结论sICAM-1、CRP、MMP-12水平升高可促使结肠癌合并糖尿病患者围术期血糖水平增加,且血糖控制情况与sICAM-1、CRP、MMP-12水平是影响术后发生感染的危险因素,并且血糖控制情况与sICAM-1、CRP、MMP-12水平四者联合检测对术后发生感染的诊断、预测价值较高。Objective To investigate the relationship between the expressions of soluble intercellular adhesion molecule-1(sICAM-1),C-reactive protein(CRP),matrix metalloproteinase-12(MMP-12)and perioperative blood glucose level in patients with colon cancer complicated with diabetes,and the predictive value of postoperative infection.Methods 98 patients with colon cancer and diabetes mellitus enrolled in Suqian Hospital Affiliated to Xuzhou Medical University from January 2020 to June 2021 were analyzed and divided into the infected group(n=40)and the uninfected group(n=58)according to whether postoperative infection occurred.The general data of the two groups were observed,the sICAM-1,CRP,MMP-12 and blood glucose levels of the two groups were compared before and 7 days after surgery,and the influencing factors of postoperative infection were analyzed by logistic regression.The predictive value of blood glucose control and sICAM-1,CRP and MMP-12 levels for postoperative infection was evaluated by the receiver operating characteristic curve(ROC).Results There was a statistical significance in perioperative blood glucose control and sICAM-1,CRP and MMP-12 levels between the two groups(P<0.05).The levels of sICAM-1,CRP,MMP-12 and blood glucose in the infected group were higher than those in the uninfected group 7 days after surgery,and the difference was statistically significant(t=36.466,8.491,8.103,2.045,P<0.05).Multivariate logistic regression analysis showed that perioperative poor glycemic control was associated with sICAM-1≥211.0μg/L,CRP<0.8 mg/L or>8.0 mg/L,MMP-12<80.0 mg/L or>140.0 mg/L level is an independent risk factor for postoperative infection,with statistical significance(P<0.05).The ROC curve results showed that the combined detection value of blood glucose control and sICAM-1,CRP and MMP-12 levels is better than each individual detection.Conclusion The increase of sICAM-1,CRP and MMP-12 levels can promote the increase of perioperative blood glucose level in patients with colon cancer and diabetes,and the co
