一次法及多次根管治疗牙隐裂伴牙髓病变的临床效果观察

摘　　要：目的 观察对牙隐裂伴牙髓病变的患者给予一次法及多次根管治疗(root canal therapy,RCT)的临床效果,对比两种治疗方式的不同。方法 选取我院2020年3月至2022年3月牙隐裂伴牙髓病变的80例患者,分为观察组和对照组,各40例,对照组牙隐裂伴牙髓病变患者给予多次RCT,观察组牙隐裂伴牙髓病变患者给予一次法RCT,分析对比观察组和对照组患者整体临床治疗疗效、术后不同阶段疼痛程度(VAS评分)、牙齿使用质量及并发症发生情况。结果 在整体临床治疗效果及治疗后30、90、100 d疼痛程度方面,观察组优于对照组,差异有统计学意义(P<0.05);术后7 d内并发症方面对比,观察组发生率低于对照组,差异有统计学意义(P<0.05);在牙齿使用质量方面对比,观察组患者牙齿使用质量各方面优于对照组,差异有统计学意义(P<0.05)。结论 一次法RCT整体临床效果较好,患者满意度高,显著改善牙髓病变患者的后期生活质量,预后良好。Objective To observe the clinical effects of single and multiple root canal therapy(RCT)on patients with cracked teeth and pulp lesions,and to compare the differences between the two treatment methods.Methods The participants in this study were selected from March 2020 to March 2022.The participants were 80 patients with dental fissure and pulp disease in our hospital,divided into an observation group and a control group,with 40 patients in each group.The control group received multiple RCTs for dental fissure and pulp disease patients,while the observation group received one RCT for dental fissure and pulp disease patients.The overall clinical treatment efficacy,pain levels at different stages after surgery(VAS score),and the comparison between the observation group and the control group were analyzed Quality of dental use and incidence of complications.Results Showed that the observation group was superior to the control group in terms of overall clinical treatment effect and pain level at 30,90,and 100 days after treatment,with a statistically significant difference(P<0.05);Aiming to compare the incidence of complications within 7 days,the observation group had a significantly lower incidence than the control group,and the difference was statistically significant(P<0.05);In terms of tooth use quality,the observation group had better dental use quality than the control group in all aspects,and the difference was statistically significant(P<0.05).Conclusions The overall clinical effect of one shot RCT is good,with high patient satisfaction,significantly improving the quality of life in patients with pulp lesions,and a good prognosis.
