全身麻醉气管插管后超声引导弓状韧带上腰方肌阻滞的腹腔镜肾癌根治术患者的术后镇痛效果

摘　　要：目的 对全身麻醉气管插管下行腹腔镜肾癌根治术的患者应用超声引导弓状韧带上腰方肌阻滞的术后镇痛效果进行分析。方法 选取2022年1月至2022年12月在广东医科大学附属医院接受治疗的182例肾癌患者,以随机数字表法均分为对照组和研究组,对照组应用常规L3入路行腰方肌阻滞,研究组应用超声引导弓状韧带上腰方肌阻滞,分析对比镇痛效果、并发症发生率等。结果 研究组NRS评分、SP、NPY、术后48 h的自控镇痛自主按压次数均低于对照组,并发症发生率低于对照组(P均<0.05)。结论 对全身麻醉气管插管下行腹腔镜肾癌根治术的患者应用超声引导弓状韧带上腰方肌阻滞,拥有显著的术后镇痛效果,有效减少并发症的发生,确保患者术后恢复的质量,减少疼痛的影响,维护患者的身心健康。Objective The postoperative analgesic effect of applying ultrasound-guided supraspinatus ligament lumbaris square muscle block in patients undergoing laparoscopic radical nephrectomy for renal cancer under general anesthesia with tracheal intubation was analyzed.Methods One hundred and eighty-two renal cancer patients who received treatment at the Affiliated Hospital of Guangdong Medical University from January 2022 to December 2022 were selected and divided into the control group and the study group by randomized numerical table method.The control group applied the conventional L3 approach to perform the lumbar-square muscle block,and the study group applied ultrasound-guided lumbar-square muscle block on the arch ligament,and analyzed and compared the analgesic effect,complication rate,and so on.Results The NRS score,SP,NPY,and the number of self-controlled analgesic autonomic presses in the study group were lower than those in the control group at 48 h postoperatively,and the complication rate was lower than that in the control group(all P<0.05).Conclusions The application of ultrasound-guided supraspinatus ligamentous lumbar flexor muscle block to patients under general anesthesia tracheal intubation for laparoscopic radical nephrectomy for renal cancer possesses significant postoperative analgesic effects,effectively reduces the occurrence of complications,ensures the quality of postoperative recovery of the patients,reduces the impact of pain,and maintains the physical and mental health of the patients.
